Abstract/Summary:
Objective: The aim of our study is to discuss the history, clinical characteristics andmanagement of the patients, investigate elements relative to the prognosis of scalpskin scar squamous cell carcinoma and explore effective ways to intervene and slowdown the progress of the disease in order to enhance the level of management andprevention thus improving the viability of the disease. Methods:We analyze the43patients retrospectively in Hunan Chenzhou’s People’sHospital in the period of2002.1~2012.1, including the onset age, history, cause, clinical course, diagnosis and management of the disease. The software of SPSS13.0was applied in the collected data by using Kaplan-Meier Product-limit method toanalyze the survival curve, Log-Rank to test and COX regression model to domultifactor survival analysis and figure out the protective and hazard factors. Results:The data collected in our study was from the cancer in natural progression. The characteristics of them are listed as follows: the number of male was larger thanfemale, a large proportion of them was over50years old, patients from cities weremore than that from countryside, the lesions at the head were mostly atrophic scar, thelesions at the top of scalp skin were mostly squamous cell carcinoma, the cause of thedisease happened in life accidents in their childhood. The way of infiltration of thecell cancer of the subjects were mostly in the way of local destruction of the scull.Histopathologic types: most of them were highly differentiated squamous cellcarcinoma. The results of Log-Rank univariate analysis,COX test and Kaplan-Meiertest showed that sex, family history, residence, pathogenic sites had no statisticsignificance on prognosis survival rate. However, the data also demonstrated that theonset age, the maximum diameter of the lesion, tumor stage and differentiation, recurrence history and the way they repair had statistical relationship with survivalrate. Conclusion:1. In addition to the pathology stage and grading which is well-known factor thatinfluence the prognosis of tumor, our study showed that the diameter of the skinlesion can be used as an important criterion in diagnosis.2. It is suggested that timely, reasonable and correct management for burn surface inthe early stage is an important way to prevent the happening of scalp skin scarsquamous cell carcinoma.3. Effective anti-scar treatment is proved to be a leading ways in the management ofthe disease.4. Make effects to stop the development of scalp skin scar ulcer should be treated askey in decreasing incidence and mortality.
