1BackgroundIn recent years,with gradually in-depth research of COPD,it is found that the clinical manifestations of COPD has a great heterogeneity,and the degree of airflow limitation can’t fully explain this phenomenon,so the phenotype of COPD has been paying more and more attention,including the study of COPD gender difference,which is one of the hot spots.The correlation between the gender and the disease also has been interpreted in TCM,there is a significant difference between men and women in both physiological function and pathological change.Even if men and women have the same kind of disease,the syndrome differentiation and treatment are different.But we didn’t find the gender difference in the TCM syndrome of COPD.2Clinical researchObjective:By the research of different genders in the record AECOPD of syndromes distribution situation,this paper discusses the TCM etiology and treatment of COPD and the influence between the sexes.Methods:Adopted cross-sectional survey, Select the hospitalized patients with AECOPD between May2010and January2012in Xiyuan Hospital, Survey general information, personal history,Chest imaging examination,pulmonary function examination,BMD examination, HAD,TCM syndrome and so on.Result:There were289patients who participated in this study, in which183cases were male and106cases were female.(1)Referring to TCM syndrome types, excess-syndrome of the male was much more than that of female, while mingling with syndromes of excess and deficiency was more for the female. It was common to observe Qi and blood stasis and liver stagnation syndrome type in female patients. Deficiency of Yin for lung and kidney in male patients was more common than female patients, and there was no statistic meaning between the male and the female for other syndromes.(2)In the aspect of TCM syndrome and frequency, accompanied symptom in female patients was mainly symptoms related to kidney, and it was common to observe symptoms relevant to heart and liver in female patients.(3)There was no statistical difference of age distribution and disease condition for AECOPD patients with different genders.(4)With respect to onset age of disease, it was earlier for male patients than that of female patients.(5)The ratio of patients who had a history of smoking for the female was lower than that of the male; there was no significant difference of COPD classification between the male and the female subjects who had a smoking history; while severity of male COPD class was less than that of the female in investigated subjects without smoking history; it had not found that whether patients had a smoking history was related to classification of COPD according to data statistics.(6)Degree of dyspnea for the female was higher than that of the male, and more severe of classification of pulmonary function, greater degree of dyspnea no matter for female patients or male patients.(7)Examination of chest imaging indicated that number of male patients with emphysema was more than that of female patients.(8)It was common for female COPD patients to be observed osteoporosis in combination with anxiety-depression compared to female patients.3Conclusion:There exists gender differences in the age of onset, complications, the clinical symptoms, the auxiliary examination, etc AECOPD patients;In TCM syndrome, female patients with COPD are more likely to merge stagnation of Liver Qi and Qi stagnation blood stasis syndrome, while lung and kidney Yin deficiency are more common in men.
