On many researches, it has always to focus on the sportsfunction injury with patients who has small vessel disease in subcortical,especially with basal ganglia ischemic damage. In recent years researchon cognitive impairment causing by subcortical small vascular diseasebecomes the focus, and whether the cognitive impairment especially thememory dysfunction caused by the basal ganglia ischemic damage can bechecked by related neural psychology and the event related potentialfeatures to help in the diagnosis, there are few relevant reports in China.This article aims to explore the variety characteristics of sensorymemory,working memory and the event related potential of patients withbasal ganglia ischemic damage, looking for an objective methods ofidentify early cognitive dysfunction especialy memory dysfunction, tosupport new basis for clinical evaluation.Methods:20patients with basal ganglia ischemic damage(d<1.5cm) andthrough regular head MRI to prove it. And those patients must beexcluded chronic ischemic cerebral white matter lesions. And they areagain according to how much the case group lesions and pathological tobe divided to groups. The case group must be out of depression throughthe Hamilton depression rating scale, and ischemia rating scale assessment, and through the daily life scale (ADL) evaluation to beconfirmed daily living skills are normal.20normal controls matched withage, sex and education. All were tried in test and complete the task:1,neuropsychological scale test----MoCA, MMSE, and phonologicalworking memory test----digital breadth test (the back and fall back);2,computer aided testing: visual-spatial working memory test (3ball loadexperiment) and sensory memory test (part of the report test);3, thedetermination of the event related potential: mismatching negativewaves (MMN) and P300. And analyse their correlation.Result: Basal ganglia lacunar infarction compares with normal controlsgroup,the MMSE score and MoCA score significantly reduced, theformer’s digital breadth test score significantly reduce, and the computeraided testing results show that the former’s visual-space working memoryin significantly extend reaction, and the ratios reduced, and the former’svisual sensory memory has longer reaction time, but no significantdecline in accuracy. The event related potential test showed that theformer’s P300and MMN have significantly longer incubation period andlower wave amplitude. Focus on the left and right group,neuropsychological scale and the event related potential all has nosignificant differences. Bilateral lesions is unilateral lesion of test displaynumbers fall back test score significantly, the visual-space workingmemory and visual sensory memory were significantly extend reaction method, P300and MMN was significantly longer incubation period,volatility decreased significantly, and the testing more than there were nosignificant difference. Compare to the single group, multiple lesionsgroup’s MoCA score significantly reduced, digital fall back scoresignificantly reduced, the ratio of visual-space working memory reduced,the respons time of visual sensory memory prolongs, P300and MMNsignificantly extend the incubation period of the volatility is decreasedobviously. P300and MMN and MoCA scale all have good correlation,P300has significant correlation with the results of visual-space workingmemory test, MMN has significant correlation with the results of visualsensory memory test. The detection rate of memory dysfunction, MMNis higher than P300, and the difference have statistical significance.Conclusions1、Patients with basal ganglia lacunar infarction has appearedcognitive dysfunction, especially in visual-space working and executivefunction, the attention and delayed memory.2、 MoCA is a goodevaluation tool in patients with basal ganglia lacunar infarction toevaluate their total cognitive dysfunction.3、 Patients with basal ganglialacunar infarction already has damage in working memory and sensorymemory, and can be reflected through the nervous psychology test.4、Theevent related potential P300and MMN can response the different aspectsof memory dysfunction of patients with basal ganglia lacunar infarction,P300can response damages of their working memory, MMN can response damages of their sensory memory, and the MMN can more early,more sensitively to response the memory dysfunction than P300.
