| Objective:In this study,we compare the surgical time,postoperative intestinal motive function change after two different kinds of biliary-enteric anastomosis through animal experiments in order to investigate which technique is more practical. And observe the transform of intestinal function and influence of traditional Chinese medicine Dachaihu decotion on it.Methods:1.By comparing different surgical experimental group dogs intestinal MMC date to assess the impact of intestinal motor function after different surgical procedures on the biliary-enteric anastomosis.The MMC date was detected by the small intestine motility detection system after surgery for3times:1month,2months and3months after surgery.2.Select20healthy adult Beagles weight8-10KG and randomly devided into4groups:input loop ligation biliary-enteric anastomosis group, Roux-en-Y biliary-enteric anastomosis group, input loop ligation biliary-enteric anastomosis and with Dachaihu decoction treatment group and Roux-en-Y biliary-enteric anastomosis with Dachaihu decoction treatment group.The traditional Chinese medicine Dachaihu decoction given from the first week after surgery since the end of the experiment.Each group take the intestinal motility function texts three times:1month,2months and3months after surgery..By comparing the intestinal MMC date of different treatment groups,assess the effect of Dachaihu decoction on the intestinal motor function after biliary-enteric anastomosis and try to find the best time to give traditional Chinese medicine.Results:1. Experiments showed that compared with the ligation input loop biliary-enteric anastomosis groups,the Roux-en-Y biliary-enteric anastomosis groups cost longer operative time,the time of MMC prolong,the time,motive index and amplitude slow down.2.Experiments showed that in the groups which given Dachaihu decoction treatment the time of intestinal MMC and I phase decreased qucickly,motive index and have grown up gradually.Compared with the groups without Dachaihu decotion, the intestinal MMC date of the Dachaihu decoction treatment groups are obviously dominant(P<0.05). 3.Experiments showed that given Dachaihu decoction treatment for3months,the MMC total time, motive index,amplitude,time of MMC Ⅲshowed no significant changes compared with the groups given for1month and2months,no statistical significance(P>0.05)Conclusions:1.Roux-en-Y biliary-enteric anastomosis cut the jejunal,destructed the intestinal nomal electrophysiogical activity, and effected the intestinal migrating motor. Input loop ligation biliary-enteric anastomosis without transected jejunum are more consistent with physiological state,cost shorter operation time stimulate body lighter and the index of MMC are better.The research shows that input loop ligation biliary-enteric anastomosis is surperior to Roux-en-Y anastomosis.2.Dachaihu decoction can improve the bowel motor function and promote recovery after biliary-enteric anastomsis.Compared with the groups not given herbal treatment,the indexs of MMCⅢ were increased in the group which given Dachaihu decoction.It can increase the time of MMCⅢ,and grown up the motive index.We could use Dachaihu decoction dialectically to cure the postoperative gastrointestinal motility dysfunction and reduce the incidence of complications after biliary-enteric anastomosis.3. Compare with the groups given herbal treatment for1months and2months,there is no significant change of MMC indicators in group given herbal treatment for3month. The best time to use traditional Chinese medicine to treat the gastrointestinal motility disorders after biliary-enteric anastomosis procedures may be in the first month after the surgery,Chinese medicine treatment is not the longer the better.4.Among all these groups,the group chose input loop ligation biliary-enteric anastomosis plus Dachaihu decoction treatment gained the best MMC index.Choose a reasonable surgical procedure and combine the use of Chinese medicine may lead to best clinical effects. |
