Objective:To study the association between the CAG repeat polymorphism in the androgen receptor and chronic heart failure.Methods:121cases with chronic heart failure were collected in research from the Second Hospital of Tianjin Medical University from October2010to December2011, and89cases without chronic heart failure were enrolled. The patients who suffered from severe insufficiency of function in liver and kidney, endocrine system disease, carcinoma of prostate and other malignant tumor were all excluded. For all selected patients, medical history including age, smoking, hypertension, diabetes mellitus(DM), cerebrovascular disease, renal function, liver function, electrolyte, lipid level and so on were taken. Routine examination of the echocardiography was performed. The CAG repeat length in the peripheral blood AR gene was examined by PCR and the direct sequencing. SPSS17.0was used for statistical processing of data.Result:1、The number of CAG repeat in all cases raged from12to31, the mean value22.34±3.283and the median22.2、The number of CAG repeat in chronic heart failure group(mean:23.13±2.751) and the control group(mean:21.26±3.639) was significant statistic difference(P<0.05).3、The CHF patients were divided into three different degree of cardiac function groups according to NYHA class. The number of CAG repeat was not significant statistic difference among three groups(P>0.05).4、Compared the index between the short AR gene group and the long AR gene group we could see the high density lipoprotein(HDL) is higher in the short AR gene group and the serum uric acid is higher in the long AR gene group.5、The short AR gene group was43.80%(53/121) in CHF and65.17%(58/89) in control, while the long AR gene group was56.20%(68/121) in CHF and34.83%(31/89)in control, and the diffenence was significant statistic difference(P<0.05).6、The HDL (r=-0.190P=0.006), the total cholesterol(r=-0.152P=0.028) and the ejection fraction(r=-0.140P=0.042) showed a significant negative correlation with the number of CAG repeat, and a significant positive correlation with serum uric acid (r=0.278P=0.000) and the left atrial diameter(r=0.157P=0.023). When we control other risk factors the HDL (r=-0.176, P=0.013) still showed a significant negative correlation with the number of CAG repeat, and a significant positive correlation with serum uric acid (r=0.237, P=0.001), but no correlation with the left atrial diameter (r=0.101, P=0.153), the total cholesterol (r=-0.131, P=0.064) and the ejection fraction (r=-0.040, P=0.571)Conclusions:1、The number of CAG repeat in chronic heart failure group was more than the control group. The CHF patients were divided into three different degree of cardiac function groups according to NYHA class. The number of CAG repeat was not significant statistic difference among the three groups. The ratio of the short AR gene group and the long AR gene group in the CHF and the control was diffenent. And the difference was significant statistic difference.2、Compared the index between the short AR gene group and the long AR gene group we could see the high density lipoprotein(HDL) is higher in short AR gene group and the serum uric acid is higher in long AR gene group.3、The HDL, the total cholesterol and the ejection fraction showed a significant negative correlation with the number of CAG repeat, and a significant positive correlation with serum uric acid and the left atrial diameter. When we control other risk factors the HDL still showed a significant negative correlation with the number of CAG repeat, and a significant positive correlation with serum uric acid.
