Objective: Vacuum sealing drainage (VSD) has been widely used in clinical for its gooddrainage effect,which can three-dimensional continuous negative pressure drainage onwound by biological supplies and sealing film with their good biocompatibility、perviousness and gas permeability. But there are problems of blockage failure and high pricein practical application process. Some researchers made different improvement on traditionalVSD and reported some good results. This study made a research on these improved VSD tofind a better one.Methods: From March2010to June2011,27patients with soft tissue defects in traumaticlower limbs, which treated with (1) traditional vacuum assisted closure technique (2)intermittent convection washing plus continuous negative pressure (3) set separated flushpipecontinuous irrigation plus continuous negative pressure (4) intermittent convection rinseplus intermittent negative pressure and the improved technology respectively,patients hadvarying degrees of skin grafting postoperatively. During the treatment we collected drainagefluid and bacterial culture. Statistics VSD accessories days, The amount of bacteria andpostoperative survival of skin area ratio of every groups.Results:Evaluation life and economic,(1) Traditional vacuum sealing drainage techniquein each piece of material with an average service life short,(3) Separate reset rinse continuous washing with continuous negative pressure was longest average life,(4)Intermittent convection wash and intermittent negative pressure(2) Intermittent convectionwash and continuous negative pressure,their treatment days somewhat less than (1),Contrast between the two was no significant difference(P>0.05). Comparison treatmenteffect and skin graft survival: Largest the amount of bacteria in method (1), but minimize inmethod (3), methods (2) and (4) bacteria in middle, comparison between the two is nosignificant differences (P>0.05). In (1)(3) the skin graft survival rate of at least and nodifferences (P>0.05), but use method (2)(4) the skin graft survival rate was significantlyhigher than (1)(2), they also had no differences (P>0.05).Conclusions: Conventional vacuum assisted negative pressure drainage techniques is themost prone to clogging,dressing to use the shortest possible time, higher risk of infectionand the highest cost. Separate reset rinse continuous washing with continuous negativepressure was longest average life, less clogging and less cost. But remove the dressing isnot better than traditional methods for skin graft survival area has a significant advantage,this may wash the local micro-bad border as a result of damage sustained relating.Intermittent convection wash plus intermittent negative pressure and Intermittentconvection wash and continuous negative pressure still have good economy andeffectiveness compared with the traditional methods, but a marked and continuous negativepressure than intermittent negative pressure is more conducive to clinical operations.Therefore, Intermittent convection wash plus continuous negative pressure haveclinical technical operation of convenience, economy and efficacy of the relativelybest, and most conducive to the promotion.
