Objective:To explore the choice of in vitro fertilization method in IVF patients with a few oocytesretrieved.Methods:The clinical data of807infertility patients with a few oocytes retrieved(≤5) suffered IVF-ETwere analyzed retrospectively. To compare the clinical efficiency of conventional IVF andintracytoplasmic sperm injection(ICSI).Result:The normal fertilization rate and available embryo rate of embryos were significantly higherwith ICSI than with conventional IVF, and the rate of multiple PN was significantly lowerwith ICSI than with conventional IVF, but the rate of high quality embryos,implantationrate,clinical pregnancy rate,cumulative pregnancy rate, live-birth rate, cycle cancellationrate and complete fertilization failure rate had favorable trend in conventional IVF groupscompared with ICSI groups,although they were not statistically significant. The patientswere divided by age and semen parameters. In patients<35years old with normal semenparameters, the normal fertilization rate and available embryo rate of embryos weresignificantly higher with ICSI than with conventional IVF,and the rate of multiple PN wassignificantly lower with ICSI than with conventional IVF, but no significant statisticallydifferences were found when compared any other clinical index between2groups. Inpatients<35years old with abnormal semen parameters, ICSI groups gave higher normalfertilization rate and lower complete fertilization rate than IVF groups, but any other clinicalindex between2groups was not statistically significant. In patients≥35years old with normal semen parameters, the normal fertilization rate was significantly higher with ICSIthan with conventional IVF, and the rate of multiple PN was lower after ICSI than afterconventional IVF, but any other clinical index were comparable between2groups;Inpatients≥35years old with abnormal semen parameters,ICSI groups produced higherfertilization rate than conventiona IVF groups, however, the rate of multiple PN, implantionrate, clinical pregnancy rate, cumulative pregnancy rate and live-birth rate were significantlylower than conventional IVF groups. Moreover, no significant statistically differences werefound in terms of any other clinical index between2groups.Conclusion:When retieved oocytes are fewer than5, ICSI can increase the normal fertilization rate andavailable embryo rate of embryos, reduce the rate of multiple PN, but cannot improve theclinical pregnancy outcome, regardless of the female age and sperm quality. Thus,ICSI can’toptimize the treatment cycles of patients, ICSI is not recommended for the patients with afew oocytes retrieved.
