Objective:In analysing the people’s health status, and the use of health services, and the demandfor health care services, setting up the needs model of people’s health management; Inanalyzing the development model of domestic health management, and foreign successfulexperience, putting forward a health management model in theoretical which is suitable forpeople at home; integrating of RFID and wireless sensor network technology (WSN) topropose the hybrid smart node (HSN), building the model and system of population healthmonitoring, and people health management model. Methods:Data acquisition methods: Data originate from field investigation and questionnaire, and the official data retrieval from network. Typical sampling method is employed to locateone country in Qujing of Yunnan province, within which sample communities are selectedrandomly, within which residents are selected randomly as surveyed people. AccordinglyLuoping of Qujing area is located as sample area,203residents are sampled within3communities, mainly aimed at understanding the demand for health care services. Thepeople’s health status data is retrievaled from the official network of Ministry of Health ofthe People’s Republic of China.Statistical analysis method include—Descriptive Statistics and Multiple Response.Nominal scales’ frequency and descriptives are used for people’s health status, and relatedchart drawn by EXCEL; Multiple Response is used for analyzing the demand of health careservices by SPSS12.0. In addition, inductive reasoning method is used to propose the demand model of healthmanagement. Results:Because of continuity and steady growth of chronic non-communicable diseases, andunaffordable the high health care costs, so that Govemments are looking for a way toimprove people’s health, and ways to reduce health care costs. The development of healthmanagement and successful cases abroad bring great opportunities to the development ofpeople health management in China. At the same time, the development and application ofhealth information technology, especially RFID and WSN technology, have proposed a newmodel for population health monitoring and health management.Population health data analysis provided by the2011National Health StatisticsYearbook, from2000to2009, nearly10years, the mortanity rate is growing, and theprevalence of chronic diseases is also steady growing, which women are higher than men,65years old and over population prevalence of chronic diseases is continue to increase,while35-64year-olds prevalence of chronic diseases shows an increasing trend. Thestatistics show that the highest prevalence of chronic diseases is circulatory system disease, the highest mortality of residents is the malignant tumor.2008Health Services Surveyshowed that two-week visiting rate, the use of hospital services and clinic attendancesincreased significantly according to2003. According to our survey data,86.7%of thepopulationwant the health care services. At the same time health costs are increasing, anincreasing of76.9%, to reach1.992135trillion yuan in2010relative to2000. From theforeign point of view, health management projects have much higher output efficiency,within5years, the benefit cost ratio of a good designed health management project is atleast between3to8.There are a variety of development models of health management through extensivereview of the literature of China, such as the cooperative model for rural residents, which iscooperated by the new rural cooperative medical system and village clinics, the cooperativemodel for urban residents, which is cooperated by community health agencies and hospital, commercial health management model and another model based on ages, etc. RFID has advantages in the accurate identification and data management, while WSNhas obvious advantages in the data monitoring and obtain. The integration of the twoadvantages to HSN, can not only meet the new demand, but also improve efficiency andreduce hardware costs. HSN is the core component of population health monitoring system, health management providers are able to analyze the health status of the monitored objectand regularly push reports by monitoring data provided by the HSN. If there is anemergency situation, a critical alert can be issued to monitored object families andproviders, of course, the monitored object can also sent SOS by pushing a key when he/shefeels critical situation. On the basis of population health monitoring, population health management modelcan implement lifestyle management, demand management, disease management, disabilitymanagement, and catastrophic care management, to reduce health costs, improveproductivity. The same time, health management projects need to be valued regularly, inorder to improve projects input and output efficiency. Conclusions:China is urgent need to change the population health status, and reduce the health costs, and to meet the demand for medical services, and to improve the health input and outputefficiency by implementing health management. The development of community healthservices has received increasing attention, and medical institutions are also developing, wecan conduct population health management by using community health services andmedical institutions. Population health monitoring system whose core component is HSNintegrated by RFID and WSN not only can provide accurate identification, and informationmonitoting capabilities to meet the demand for population health monitoring, but alsoimprove data transmission efficiency, and lower utility costs. The health managementmodel which is based on the health monitoring system, and health management strategies, and regular project evaluation is suitable for us.
