Psoriasis is a common chronic inflammatory skin disease characterized by epidermis over-proliferation, cellular hyperkeratesis, parakeratosis, and associating with inflammatory infiltration with a complicated pathological process and the cause of psoriasis is not fully understood yet.Intercellular adhesion effects stemming from different cell junctions of epithelial cells. Rigorous regulatory mechanism exists with the components and functions of cells junctions, which depends on interactions of various adhesive cells. P-cad plays a mediation role for cells of same affinity and intercellular calcium dependence protein cadherin.P-cad expression weakening or deletion causes intercellular mutual adhesion force reduction which leads to loose cell junction and cell over-proliferation.NE, which plays a key role in the course of inflammation occurrence, is an important protease of PMNs. It is found by study that over-expression of NE results in tissue lesion and inflammation generation as well as cell proliferation.Objective:The aim of this review is to detect expression of NE and P-cad in patients with psoriasis vulgaris and analyse correlation between expression levels of NE&P-cad and states of illness, further discussion of potential pathogenic factors of psoriasis is performed, and which provides new thoughts for explorations of etiological factor, pathogenic mechanism and psoriasis therapy.1、In order to observe the expression levels of P-cad and NE in serums of patients with psoriasis vulgaris and their relationship with etiological factors, serums taken from patients with psoriasis vulgaris are detected and compared with that of normal controls.2、In order to further observe the expression levels of P-cad in skin lesions of patients with psoriasis vulgaris and their relationship with etiological factors, skin lesions of patients with psoriasis vulgaris are detected and compared with that of normal controls.Materials:1、Patients group All together40cases of patients with psoriasis vulgaris, of which16progressive phase,15stationary phase and9involuting phase, are taken from outpatient clinic of department of dermatology at Subei People’s Hospital and first People’s Hospital in Yangzhou city from May2010to March2011. Average age is34.75of all the cases of35.78±15.62, and average course of disease is5.63years of all the cases of7.34±4.28.2、control group40cases of male26and female14with average age of28.78of all cases aging from33.26±11.35at cosmetic plastic surgery and health-care center of dermatologic department of Subei People’s Hospital and first People’s Hospital of Yangzhou city.3、Inclusion criteria Cases collected shall exclude of autoimmune diseases of lupus erythematosus, pemphigus, dermatomyositis, rheumatic arthritis and other severe systematic diseases, and no large dosage of immune inhibitors and corticosteroids affecting immunologic function are taken within three months; none of other systematic and immunologic diseases in addition to above mentioned diseases either. The control group shall exclude of all above mentioned diseases and psoriasis as well.Methods:1、PASI evaluation is performed to patients group2、Detection is performed by means of euzymelinked immunosorbent assay to expression levels of NE and P-cad in serums of patients group and control group. 3、Detection is performed by means of immunohistochemical method to P-cad expression levels of both skin lesions of patients group and tissues of control group.4、Statistics softwares of SPSS13.0and Excel2003are applied for results collection and analysis.Results:1、Expression level of P-cad in psoriasis vulgaris serums is significantly higher than in normal controls (P<0.01), expression level relates to severity of illness, which is a positive correlation.2、Expression strong of P-cad in skin lesions of psoriasis vulgaris while healthy control group is just expressed in basal layer of epidermis3、Expression Level of NE in psoriasis vulgaris serums is significantly higher than in normal controls (P<0.01), expression level relates to severity of illness, which is a positive correlation.Conclusion:1、P-cad The tests show that P-cad may participate in the psoriasis generation, also shows structural and functional abnormality of epidermal cells junction in psoriasis patients, and this may be caused by destruction of some factors. NE, as an important protease of PMNs associating with PMNs infiltrating, has a destructive effects to extracellular matrix and numerous protease. Thus, it is concluded that NE probably participate in the decomposition and destruction to P-cad. Detection on changes of P-cad content in serums and skin lesions is beneficial to psoriasis assessment and prognosis. Restore P-cad to normal expression level by means of eliminating the destructive factors may have certain effects to psoriasis treatment.2、NE It is concluded that NE may have close band with psoriasis vulgaris generation, also shows that inflammatory response exists in psoriasis vulgaris with NE plays a significant role in the response. Inflammation mediation role of NE is capable of tissue destroying and micro-abscess formation. Vicious circle of inflammatory response may form due to its correlation with PMNs which may lead to inflammatory changes as vasodilatation and permeability strengthenment associating with further damages to tissues. All of which may closely relate with inflammation generation of psoriasis vulgaris. Interaction of NE with other factors can accelerate cell proliferation which is in line with psoriasis histologic performances. Detection of NE levels of serum in patients have certain reference effects to evaluation of psoriasis severity; active therapy to reduce or eliminate the exorbitant NE expression, may benefit psoriasis inflammation response and psoriasis treatment.
