ObjectiveDiscuss and compare the malignant pleural effusion and tuberculousinflammatory pleural effusion of PTN of Survivin, of VEGF protein and themRNA expression, as well as its joint cytology the sensitivity of the judgment ofmalignant pleural effusion. Become one of the new method of clinicaldiagnosis of malignant pleural effusion.MethodsEnzyme-linked immunosorbent assay (ELISA), RT-PCR method to detectJune2011to February2012to collect50cases of cancer patients withmalignant pleural effusion specimens, and the same period in28cases oftuberculous pleural effusion specimens, inflammatory pleural effusionspecimens in PTN, Survivin, VEGF protein and mRNA expression, combinedwith cytology results comparative analysis. Analysis of different groups ofhydrothorax between PTN, Survivin, VEGF protein and mRNA expression di-fference.Results1, PTN gene in cancer with pleural effusion and non cancerous pleuraleffusion in the expression rates were (55.57+2.400) ng/ml,(3.719+1.827) ng/ml; differential diagnosis of benign and malignant pleural effusion of thesensitivity, specificity, correct index is respectively93.3%68.6%and0.619, P= statistical analysis in0.038, with significant statistical significance;2, Survivin gene in cancer with pleural effusion and non cancerous pleuraleffusion in the expression rates were (65.57+2.201) ng/ml,(2.107+1.109) ng/ml; differential diagnosis of benign and malignant pleural effusion of thesensitivity, specificity, correct index is respectively95.3%84.2%and0.795; P=statistical analysis in0.013, with significant statistical significance;3, VEGF gene in cancer with pleural effusion and non cancerous pleuraleffusion in the expression rates were (54.43+2.190) ng/ml,(3.690+1.701) ng/ml; differential diagnosis of benign and malignant pleural effusion of thesensitivity, specificity, correct index is respectively92.2%78.2%and0.705; P=statistical analysis in0.021, with significant statistical significance;4, PCR product semi quantitative analysis: PTN, Survivin, VEGFmRNA inmalignant pleural effusion cytology positive expression were0.5733+0.1764,0.310+0.098,0.6773+0.1663; cytologically negative expression were0.5601+0.1755,0.309+0.096,0.6782+0.1654; in tuberculosis, inflammatorypleural effusion is expressed as0.2961+0.0845,0.2657+0.0342,0.2852+0.0734;0.2989+0.0723,0.2546+0.0453,0.2752+0.0734. Malignant pleuraleffusion of PTN, Survivin, the positive expression rate of VEGFmRNA wassignificantly higher than that of tuberculosis, inflammatory pleural effusion (P <0.05);5, No significant differences in the cytology-positive malignant pleuraleffusion group with negative cytology of malignant pleural effusion group ofPTN and Survivin, of VEGF protein and mRNA levels (P <0.05);6, linear correlation analysis showed that PTN, Survivin, VEGF positivecorrelation (R2=0.9634, P <0.05)ConclusionsApplication of enzyme linked immunosorbent assay (ELISA) and RT-PCR detection assay for the detection of pleural effusion in PTN, Survivin, VEGFprotein and mRNA expression in determining cancer pleural effusion has a sensitivity and specificity of diagnosis of cancer, may be used as a new index.Especially for negative cytology of malignant pleural effusion in patients withdifferential diagnosis and therapy.
