Objective:To investigate the effects of cardiomyocyte apoptosis on left ventricular remodeling with acute myocardial infarction (AMI) model of Wistar rats and the effects of early fluvastatin and Qishengyiqi extractive intervention on left ventricular remodeling and explore the potential mechanism.Methods:1.The70male Wistar rats were randomly divided into5groups:the control group(A, n=10) rats were fed routinely7days without medicine intervention;sham operation group(B,n=12)rats were fed routinely7days before sham operation without medicine intervention;the acute myocardial infarction group(C, n-18) rats were fed routinely7days before AMI operation without medicine intervention;the Qishenyiqi extractive group(D, n=15) rats were treated with Qishenyiqi extractive100mg/Kg/d7days before AMI operation; the fluvastatin group(E,n=6), rats were treated with fluvastatin4mg/Kg/d7days before AMI operation.2.The experimental AMI model of Wistar rats was established by ligation of left anterior descending branch of coronary artery.3.Hemodynamic evaluation were underwent just24h after coronary ligation or sham operation and recorded the following parameters:HR, SBP, DBP, MBP, LVSP, LVEDP,+dp/dt max,-dp/dt max.4.Rats were killed after hemodynamic evaluation. The heart samples were collected and used for histological, terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling (TUNEL) and immunohistochemical. Calculate myocardial apoptosis index (MAI), Mean Density (MD) of Bcl-2and Bax protein and the ratio of Bcl-2/Bax.Results:1.A11rats in groups A and B (n=12) were survrived;9rats were survived in group C, the success rate of operations was50%;9rats were survived in group D, the success rate of operations was60%;12rats were survived in group E, the success rate of operations was80%. There were no significant differences in regard to survival rate among the three groups (P>0.05).The success rate of all AMI operations was62.5%.2.The SBP,MBP,+dp/dt max,-dp/dt max in AMI model groups were significantly lower than those in group A and B (P<0.05);those in group D and E were higher than those in group C (P<0.05), There were no statistical differences between group D and E (P>0.05); The LVEDP in AMI model groups were significantly higher than those in group A and B (P<0.05);compared with group C, the LVEDP in group D and E were significantly reduced(P<0.05), there were no statistical differences between group D and E(P>0.05).3.The MAI of infracted region and border zone in AMI model groups were significantly higher than that in group A and B (P<0.05), compared with group C, the MAI of infracted and border zone in group D and E were significantly reduced (P<0.05), there were no statistical differences between group D and E (P>0.05). The MAI of non-infracted region in group C was higher than that in group A and B (P<0.05), there were no significantly differences among the rest groups (P>0.05).4.Compared with group A and B, the Bcl-2protein in the AMI model groups in infracted region and border zone were increasd (P<0.05); compared with groupC, the Bcl-2protein of the group D and E were significantly increased in infracted region and border zone (P<0.05), there were no statistical differences between group D and E(P>0.05).5.The Bax protein in AMI model groups in infracted region and border zone were significantly higher than those in group A and B,(P<0.05);compared with group, C,the Bax protein in group D and E were significantly reduced in infracted region and border zone (P<0.05),there were no statistical differences between group D and E(P>0.05).6.In infracted region and border zone, the ratio of Bcl-2/Bax in group C was significantly lower than that in group A and B (P<0.05), the ratio of Bcl-2/Bax in group D and E were obvisoly higher than those in group C (P<0.05),there were no statistical differences between group D and E (P>0.05).In non-infarction region, the ratio of Bcl-2/Bax in group C was lower than that in group A and B (P<0.05),there were no significantly differences among the rest groups(P>0.05).Conclusions:1.There was amount of myocardium cell apoptosis in infacted region and border zone after AMI24h in rats, the expression of Bcl-2and Bax protein in infacted region and border zone were increased, the ratio of Bcl-2/Bax was reduced.2.Short-term pre-intervention with fluvastatin and Qishengyiqi extractive could reduce myocardium cell apoptosis,Fluvastatin and Qishengyiqi extractive may be through pro-expression of Bcl-2, anti-expression of Bax and increasing Bcl-2/Bax ratio resist apoptosis.3.Short-term pre-intervention treatment with fluvastatin and Qishengyiqi extractive can reduce myocardium cell apoptosis, therefor improved cardiac fucution and the ealy left ventricular remodeling.
