Objective: Periosteal protein(POSTN)is an adhesive protein that participates in the process of ventricular remodeling after acute myocardial infarction(AMI)and regulates protein molecules.This topic aims to observe and quantitatively analyze the relationship between serum POSTN levels and left ventricular wall motion score index(LVWMSI)and other parameters in patients with AMI before percutaneous coronary intervention(PCI),to preliminarily evaluate the short-term ventricular remodeling and prognosis after PCI to explore the potential value of serum periosteal proteins for it.Method: During the period from December 2021 to August 2022,a total of 78 patients who experienced AMI within 24 hours and successfully received PCI treatment at the Second Hospital of Hebei Medical University were selected.Among them,there were 45 male cases and 33 female cases,with an average age of 62.53 ± 12.22 years.Before PCI,the serum POSTN level of patients was measured,and each patient was grouped according to the median serum POSTN.The patients were divided into group A with high POSTN(≥ 122.0 pg/m L)and group B with low POSTN(<122.0 pg/m L).Before PCI,cardiac ultrasound was performed,and data such as left ventricular ejection fraction(LVEF)and left ventricular end diastolic volume(LVEDV)were measured and recorded in detail,and the left ventricular wall motion integral index(LVWMSI)was measured and recorded in detail after calculation.To explore the correlation between LVWMSI and POSTN in both groups of patients.At the 6th month after PCI,a cardiac ultrasound review is required to record relevant data such as left ventricular end diastolic volume(LVEDV).Specifically,the LVEDV reexamination result for the sixth patient was LVEDV1,the LVEDV before PCI was LVEDV0,and the left ventricular end diastolic volume growth rate was △ LVEDV%=(LVEDV1LVEDV0)/LVEDV0 × 100%。 All patients were divided into ventricular remodeling group(△ LVEDV ≥ 20%,n=36)and non ventricular remodeling group(△ LVEDV<20%,n=42)based on left ventricular end diastolic volume growth rate.Further research is needed on the risk factors for ventricular remodeling and the role of serum POSTN levels in early prediction of ventricular remodeling.Results:1.There is a significant correlation between serum POSTN level and left ventricular wall motion integral index(LVWMSI)after myocardial infarction(r=0.798,P<0.01),indicating a close correlation between the two;2.One of the possible risk factors for recent ventricular remodeling after myocardial infarction is high serum periosteal protein levels;3.The serum POSTN level can be used as a predictor of heart ventricular remodeling in patients after myocardial infarction 6 months after PCI.Using receiver operating characteristic(ROC)curve analysis,with an area under the curve of about 0.804,P<0.05;At a cutoff value of 126.63pg/ml,the sensitivity and specificity for predicting ventricular remodeling were approximately69.4% and 81.0%,respectively.Conclusions:1.There is a correlation between serum POSTN levels and ventricular wall motion after myocardial infarction.2.High serum POSTN levels are a risk factor for the recent occurrence of ventricular remodeling after PCI in myocardial infarction and may have a predictive effect on ventricular remodeling.
