ObjectivesColorectal cancer, easily metastasis to liver, is one of the most popular digestivesystem malignant tumors.Liver metastasis is one of the most important clinical prognosticfactors. Only10%-20%patients had chance to accept metastatic tumor resection, althoughoperation is the curative treatment.Transcatheter arterial chemoembolization (TACE)therapy is important in treating liver tumor and it has been extensively applied to treat livermetastatic colorectal cancer. The objective of this study is to evaluate the therapeuticefficacy and to identify the prognostic factors of TACE in the patients with colorectal livermetastases (CRLM).MethodsThe clinical data of47patients with CRLM treated with TACE were retrospectivelyreviewed.Clinical characters such as gender, age, Synchronous or metachronous livermetastases, original tumor location, extrahepatic metastases, baseline CEA level, thedistribution of liver metastases, metastatic tumor number and size; and therapy factors suchas TACE times, tumor blood supply, tumor lipiodol deposit, primary tumor resection,were included in the analysis.Univariate survival analysis of Kaplan-Meier andmultivariate survival analysis of Cox regression were used to evaluate the prognosticfactors.Results1.The median survival time was20months, and the0.5-, l-,2-,3-year survival rateswere91.5%,74.5%,37.1%,10.1%, respectively.2.Univariate analysis showed that the following factors were related with theprognosis of the CRLM patients who had undergone TACE: baseline CEA level,extrahepatic metastases, metastatic tumor number, the distribution of liver metastases,pathological type, Differentiation degree, original tumor location,gender, TACE times, primary tumor resection, tumor blood supply, tumor lipiodol deposit.3.Multivariate analysis indicated some independent prognostic factors for the CRLMpatients after TACE treatment: pathological type,primary tumor resection,Differentiationdegree,TACE times,tumor blood supply,without extrahepatic metastases related to goodsurvival.ConclusionsTranscatheter arterial chemoembolization is an effective therapy for colorectal livermetastases. Patients with tumor blood supply, differentiation degree, primary tumorresection, pathological type, more TACE times, without extrahepatic metastases wouldhave good survival. |
