ObjectiveMonitoring fatal Middle cerebral artery(MCA)and renal artery(RA)by thetechnology of color Doppler flow imaging(CDFI) to predict fetal intrauterinehypoxia of pregnancy induced hypertension (PIH).MethodsCDFI was used to detect the blood flow velocity waveforms of fetal middlecerebral artery(MCA) and renal artery (RA) in78cases of normal latepregnancy and81cases in which the number of fetal intrauterine of PIH were49cases,and32cases which were not fetal intrauterine. The pulsatility index (PI),resistant index (RI) and peak systolic to end diastolic ratio (S/D) of MCA and RAwere calculated. All the fetuses would be visited pregnancy prognosis until thepregnancy procedure end.All these results were compared with some clinicalindex of hypoxia,such as the abnormal Apgar scores,low weight neonate,fetaldistress,neonatal asphyxia and neonatal death.ResultsCompared with the normal pregnancy group, the hypertension fetalintrauterine group had lower levels of MCA and the difference hadsignificance(P<0.01),and obviously higher levels of RA resistance index andthe difference had significance (P<0.01). Compared with the normal pregnancygroup, the hypertension group had lower levels of MCA and the difference had significance(P<0.05),and obviously higher levels of RA resistance index andthe difference had significance (P<0.05). Compared with the hypertensiongroup, the fetal intrauterine group had lower levels of S/D、RI of MCA and thedifference had significance(P<0.01), and obviously higher levels of RAresistance index and the difference had significance (P<0.01),but the PI ofMCA had no difference(P>0.01).In the cases of the fetal intrauterine group,S/D、PI、RI of MCA wereinverse correlated to those of RA and the difference had significance (P<0.01).In the cases of the hypertension group,S/D、PI、RI of MCA were inversecorrelated to those of RA and the difference had significance (P<0.01).The adverse perinatal outcome incidences in the hypertension group weresignificant higher than that of normal pregnant group and the difference hadsignificance (P<0.01). The adverse perinatal outcome incidences in the fetalintrauterine group were significant higher than that of normal pregnant groupand the difference had significance (P<0.01).Compared with the normal pregnancy group, the hypertension group hadlower levels of the Apgar score, amniotic fluid turbidity,fetal heart rate and fetalmovement, and the difference had significance(P<0.01). Compared with thenormal pregnancy group, the fetal intrauterine group had lower levels of theApgar score,, amniotic fluid turbidity,fetal heart rate and fetal movement, andthe difference had significance(P<0.01). Compared with the hypertensiongroup, the fetal intrauterine group had no obvious difference of the Apgar score,(P<0.05).ConclusionIn the cases of the fetal intrauterine group and the hypertension group,S/D、PI、RI of MCA were inverse correlated to those of RA. The Apgar score,amniotic fluid turbidity,fetal heart rate and fetal movement could indicate thefetal intrauterine. Compared with the normal pregnancy group,the resistance index of MCA and RA of the hypertension group and fetal intrauterine group hadlower levels. CDFI can accurately monitor the blood flow data of MCA and RA ofthe fetus in PIH and can supply important information about the fetal braincirculation and the fetal surrounding circulation and can predict the status ofintrauterine hypoxia, which may play an essential role in predicting pregnancyprognosis by early detection and prevention and treatment of fetal distress intime....
