Background: Fetal growth restriction is one of the common complications of perinatal period,such newborns even survived their childhood physical development is also slower than normal children,and may be accompanied by mental retardation.Color Doppler flow imaging(CDFI)can be used to obtain FGR fetal circulation information more convenient in obstetric,its assessment of fetal growth restriction and determine the value of its prognosis of pregnancy also received the attention and recognition of clinicians,which pregnant Advanced ultrasound can monitor blood flow parameters to determine whether the emergence of intrauterine hypoxia and to understand its severity,and then for the clinical termination of pregnancy to provide effective basis.Objective: To evaluate the clinical value of color Doppler ultrasonography in assessing fetal growth restriction and judging pregnancy outcome in pregnant women with umbilical artery,middle cerebral artery and aortic arch.Methods:180 cases of pregnant women with maternal and neonatal ultrasonography was selected in our hospital.Among them,52 cases of fetus with limited intrauterine growth were diagnosed as FGR group.128 cases with no abnormalities were set as a control group.Two groups of pregnant women were pregnant 34 to 36 weeks.The peak systolic velocity(PSV),diastolic velocity(EDV),pulsatility index(PI),resistance index(RI)and PSV/ EDV(S/D)value were measured by color Doppler ultrasonography(UA)and middle cerebral artery(MCA).The peak systolic velocity(PSV)and systolic reflux velocity(ESRV)of Aol were measured.The differences of blood flow parameters between umbilical artery,middle cerebral artery and aorta were compared between the two groups.To evaluate the value of blood flow parameters of umbilical artery,middle cerebral artery and aortic ganglion in diagnosing fetal intrauterine growth restriction and judging pregnancy outcome.Results:The incidence of perinatal abnormal pregnancy outcome in FGR group was significantly higher than that in control group(P <0.01).The parameters of UA S / D,PI and RI in FGR group were significantly different from those in control group(P<0.05).There was no significant difference in PSV value between the two groups(P>0.05).There were significant differences in the parameters of MCA S / D,PI and RI between the two groups(P <0.05).There was no significant difference in PSV value between the two groups(P <0.05).The ESRV of FGR group was higher than that of the control group(P < 0.05).The S / D,RI,PI values of the UA and MCA and the ESRV of Ao I were analyzed by ROC curve.Each index had the ability to identify the control group and FGR group,and the optimal diagnostic value was obtained by ROC curve.Patients with FGR group were divided into high and low expression groups(UA-RI high vs UA-RI low;MCA-RI high vs MCA-RI low and Ao I-ESRV high vs Ao I-ESRV low)by the optimal diagnostic value of UA-RI = 0.66(Sensitivity = 0.90;specificity = 0.75),MCA-RI = 0.69(sensitivity = 0.88;specificity = 0.77)and Ao I-ESRV = 35.5(sensitivity = 0.85;specificity = 0.94),respectively.and to analyze intrauterine hypoxia and to predict the outcome of pregnancy outcomes of FGR pregnant women in the high and low group,there was significant difference in MCA-RI group and Ao I-ESRV group,and no significant difference in UA-RI group.Conclusion:S / D,RI and PI values in umbilical artery and middle cerebral artery are sensitive markers to evaluate FGR in late pregnancy,as well as ESRV values in aortic ganglion;prenatal Doppler ultrasonography umbilical artery,middle cerebral artery,Aortic ganglion blood flow parameters combined diagnosis of fetal growth and development limited can improve the accuracy,and has important clinical value.
