Differences Between Asthma With Allergic Rhinitis And Asthma Alone

There are a lot of similarities between bronchial asthma and allergic rhinitis fromepidemiology、etiology、pathogenesis、treatment,etc. Both of them are allergic respiratoryillness, they often coexist at the same time, and influence each other. In2001,when the worldhealth organization (WHO) organized otolaryngology experts around the world to discusshow allergic rhinitis (AR) impacted on asthma (AS)(ARIA),they put forward the view of“one airway, one disease”.There are a lot of literatures about the relationship of asthma andallergic rhinitis.A lot of literatures point out that if we give special treatment of rhinitis whenwe treat asthma, it will get much more efficient. Thus, are there differences between themfrom family history、allergic history、asthma control test (ACT)、auxiliary examination、therapeutic drugs and effect of treatment,etc? In the group of asthma with allergicrhinitis,how many people are taken measures to treat rhinitis? Although there are somereports on these aspects at home and abroad, but it is not systematic and comprehensive. Theaim of the study is to give a further research about the differences from asthma with allergicrhinitis and asthma alone, we hope it can provide theoretical basis for clinical work abouttreatment of respiratory allergic disease from the whole breathing system and improve theclinical curative effect.【Objectives】Through the clinical data,we study on the differences of bronchial asthma with allergicrhinitis and bronchial asthma alone from family history、allergic history、asthma control test(ACT)、laboratory indexes、effect of treatment,etc. we hope it can provide theoretical basisfor clinical work about treatment of respiratory allergic disease from the whole breathingsystem and improve the clinical curative effect.【Methods】We select100patients with bronchial asthma from February2011to January2013whom treated in Sino-Japanese Friendship Hospital and divide them into two groups:the oneis asthma with allergic rhinitis(49persons),the other is asthma alone(51persons).Wecompare with the differences of them from family history、allergic history、incentive、 occurrence season、occurrence time、first generant age、the course of disease、the days inhospital、the times in hospital in recent one year、symptom、FEV1/FVC%、FEV1%Pred、blood gas analysis、routine blood test、the effect of treatment,etc.【Results】1、In general:in the asthma with allergic rhinitis group,there are20man and29women,the average age is (46.69±14.88), the average height is （161.13±8.14）cm, theaverage weight is（54.35±8.73）Kg.In the asthma alone group, there are21man and30women,the average age is (49.40±11.00), the average height is （162.22±7.92）cm, theaverage weight is（55.16±7.62）Kg. No statistical differences (P＞0.05) are revealed fromage、gender、height、weight、family history、incentive、occurrence season and the numberof severe asthma patients.In the asthma with allergic rhinitis group,there are15persons whohave a clear allergic history, the average number of days in hospital is (11.35±3.52),10persons’ inpatient number are more than2times in recent one year, the average course ofdisease is (11.98±8.77) years, the average first generant age is (34.92±14.05),17persons’occurrence time is at night and in the morning. In the asthma alone group, there are7persons who have a clear allergic history, the average number of days in hospital is(9.41±2.66),3persons’ inpatient number are more than2times in recent one year, theaverage course of disease is (8.16±10.39) years, the average first generant age is(41.01±14.55),6persons’ occurrence time is at night and in the morning. There is significantstatistical difference (P＜0.05) between them.2、Asthma control test(ACT): compared with the asthma alone group,the asthma withallergic rhinitis group have more serious clinical symptoms and poorer asthma controlsituation.There are significant statistical differences (P＜0.05) between them.3、Laboratory indexes: in the asthma with allergic rhinitis group,the mean valueof FEV1/FVC%is54.01%±11.93%，the mean value of FEV1%Pred is51.11%±15.98%，the mean value of PaO2is65.31±13.89mmHg,the mean value of the percentageof eosinophil is8.98±4.15%; In the asthma alone group, the mean value of FEV1/FVC%is61.43%±12.72%，the mean value of FEV1%Pred is56.96%±13.11%，the mean value of PaO2is71.00±13.03mmHg,the mean value of the percentage of eosinophil is7.10±5.01%. There are significant statistical differences (P＜0.05) between them.4、The effect of treatment: in the asthma with allergic rhinitis group, there are36persons who use the intravenous drip of hormone(methyl prednisolone),11persons usehormone dosage of40㎎/d,19persons use hormone dosage of80㎎/d,6persons usehormone dosage of160㎎/d,7persons use one kind of anti-inflammatory drugs,19personsuse two kinds of anti-inflammatory drugs,23persons use three kinds of anti-inflammatorydrugs, there are14persons whose number of days completely or obviously required less than3days, there are27persons whose number of days completely or obviously required lessthan1weeks, there are8persons whose number of days completely or obviously requiredmore than1weeks. In the asthma alone group, there are27persons who use the intravenousdrip of hormone(methyl prednisolone),19persons use hormone dosage of40㎎/d,7personsuse hormone dosage of80㎎/d,1persons use hormone dosage of160㎎/d,25persons useone kind of anti-inflammatory drugs,18persons use two kinds of anti-inflammatory drugs,8persons use three kinds of anti-inflammatory drugs, there are26persons whose number ofdays completely or obviously required less than3days, there are22persons whose numberof days completely or obviously required less than1weeks, there are3persons whosenumber of days completely or obviously required more than1weeks. There are significantstatistical differences (P＜0.05) between them.In the asthma with allergic rhinitis group,there are14persons who use nasal spray in the treatment, accounted for28.57%.【Conclusion】Compared with the asthma alone group,people in the asthma with allergic rhinitis grouphave more allergic history、more days in hospital、more inpatient number in recent one year、earlier generant age、more course of disease、more occurrence time which is at night and inthe morning.The severity of symptom（sasthma control test）、the change of PaO2、the changeof FEV1/FVC%and the change of FEV1%Pred and the effect of treatment of the asthmawith allergic rhinitis group are worse than the asthma alone group. Eosinophil infiltrationincreases more significantly.The part with treatment of rhinitis in the asthma with allergicrhinitis group is accounted for28.57%.
