Objective:1、The research about that radiofrequency RFTC on trigeminalnerve semilunar ganglion treats trigeminal neuralgia under three-dimensionalCT image-guided.2、 Clinical study of therapeutic effect about thatradiofrequency RFTC on trigeminal nerve semilunar ganglion treats trigeminalneuralgia by local anesthesia analgesia outside the oval foramen.3、Clinicalstudy of complications about that radiofrequency RFTC on trigeminal nervesemilunar ganglion treats trigeminal neuralgia.Methods: By the guidance of three-dimension CT with anterior approachof Hartel’s technique, under the local anesthesia where has not yet entered theforamen ovale, we percutaneously punctured the oval foramen to gasserianganglion and performed radiofrequency thermocoagulation on177cases ofprimary trigeminal neuralgia, observe analgesic effect during the operation and analysis the the complications happened after theoperation.Result: There was no pain in158cases during the RFTC, the other16cases were caused mild pain that they could endure, the rate of pain relief was100%.24hours after RFTC,3cases of primary trigeminal neuralgia were notcured, and the total effective rate of this operation was98.31%.4casesrecurrence was found with3months of follow-up, the recurrence rate was2.26%;4cases were found in6months follow-up, the recurrence rate was2.56%; in12month, there are5cases had recurred, the recurrence rate was5.00%.Conclusion: In this study, treating trigeminal neuralgia underthree-dimensional CT image-guided is scientific, accurate, shorter the time ofpuncture, reducing puncture damage. The therapeutic effect of local anesthesiaanalgesia outside the oval foramen is precise and safe. The patients who havedifficult disease, older age and on critical condition could be treated by this way.The number of complications about the present method treating177patientswith trigeminal neuralgia was significantly reduced as compared to the othermethods. |
