Background: To identify and reverse ischemia penumbra timely is theprecondition of favorable prognosis when suffering from Acute ischemic stroke. Topromote early recanalization and collateral circulation establishment is the key toachieve good prognosis. At present, the commonly used methods which assessingischemia penumbra including positron emission tomography imaging technique,single photon emission computed tomography (CT) imaging technique, CT perfusionimaging, magnetic resonance perfusion weighted imaging, etc. But the aboveinspection methods are to use radioactive nuclide or contrast agent, the price is higherand the scanning time is long. All these disadvantages limit the clinical application forsome patients. Susceptibility weighted imaging(SWI) is to avoid the aboveshortcomings, and maybe has a broad clinical application prospect.Methods: Eighteen patients, who were diagnosed Acute ischemic stroke in thefirst hospital of Jilin university from January2012to January2012, were collected.They had new lesions on the DWI as the research object. The patients madeinspection including head DWI, PWI, and SWI related imaging examination. Afteradmission to perfect the related imaging examination, respectively do ASPECTS onDWI, SWI, PWI, because Prominent vein (PV) can be more clear display in the mIPas the minimum density projection in SWI, therefore the early Albert CT grading ofSWI is mainly the scores of mIP. PWI can reflect the blood perfusion of theorganization, can through the following several parameters, for example,TTP、MTT、CBF and CBV, using computer software processing, finally analysis and judge them.MTT is an important parameter of PWI, is also sensitive can be used to reflect theindex of ischemia, basically can reflect the scope of ischemia area, so we in the earlyof PWI Albert CT grading, mainly to score determined by MTT, then comparing theSWI-DWI (mIP-DWI) mismatched range and PWI-DWI (MTT-DWI) canmismatched range comparison and doing statistical analysis. To find out the Prominent Vein (PV) around the lesion area on the SWI as the Voxel of Intresting, tomeasure and record the cerebral blood volume (CBV) on the MRP correspondingcontralateral image region.To study the correlation and to make the statistical analysisbetween the CBV and PV. Statistical methods: SPSS17.0data analysis software, anddocimastic significance level was p<0.05.Results:(1)The study shows that the SWI-DWI and the MTT-DWI does matchin the same range, no statistically significant difference (P=0.187) both in theassessment of ischemic penumbra role.(2) The grade of PV and the CBV of theipsilateral brain tissue on admission have statistically significant differences(P=0.046).(3) MRA as a reference standard, found vascular magnetic sensitivecharacter can be judged responsible blood vessels on the SWI.Conclusions:(1) SWI-DWI can evaluate the ischemic penumbra range.(2) PVmay reflect the increased blood volume of the lesion side of the brain tissue.(3) SWIcan be found the thrombosis of the responsibility vascular.
