Objective: To evaluate the value of chest MRI in the detection ofrib metastasis in lung cancer, especially in the preoperative detection.Methods: A retrospective study was carried out in54patients inour hospital with a histopathological and/or cytologic diagnosis of lungcancer,and who had all undergone bone scintigraphy, chest CT and chestMRI. We compared the sensitivity, specificity and accuracy of differentkinds of imaging methods for the detection of rib metastases on aper-patient and per-lesion basis by χ2test.Results: Of the54patients,135rib lesion was considered to besuspicious of a metastasis. Based on the results of initial and follow-upbone scintigraphy, CT, and MRI examinations and pathologicalexaminations,30patients (120lesions) were confirmed to be ribmetastases. The30patients who confirmed to be rib metastases were allmultiple rib metastases. The sensitivity and accuracy of MRI weresignificantly higher than those of bone scintigraphy and CT on a per-lesion basis (P=0.001, P=0.001, both P<0.05). In the threeexaminations methods correctly detected rib metastasis consistency. On aper-patient, in the54cases,45cases (83.3%) in the MRI and bone scan,CT is consistent.45cases (83.3%) in the bone scan and CT is consistent.5cases (9.3%) were detected correctly by MRI alone. On a per-lesionbasis, in the135lesions,46lesions (34.1%) in the MRI and bone scan,CT is consistent.48lesions (35.6%) in the bone scan and CT isconsistent.40lesions (29.6%) were detected correctly by MRI alone.Conclusion: MRI is superior to bone scintigraphy and CT for thedetection of rib metastases, MRI is necessary for the screening, diagnosis,and monitor of ribs metastasis in lung cancer.
