Objective:To investigate the relativity of the ADC value of magnetic resonance diffusion-weighted imaging in pathological grades of IgA nephropathy.And provide a relevant reliable useful in clinical basis on diffusion-weighted imagingMethods:Diffusion weighted imaging(DWI) of kidneys was performed in73persons were included20normal volunteers without kidney disease, hypertension, and taking kidney damage drug history and53patients who received renal biopsy was detemined as graded Ⅰ,Ⅱ,Ⅲ,Ⅳ and Ⅴ with IgA nephropathy by Hass classification system.The ADC value acquired on ADC images.Every kidney select three levels which are renal higher pole,hilum plane and the lower pole, each level randomly selected the renal medulla as possible as close to the cortex,then measure ADC values in about60mm2three regions of interest (ROI) area,and avoid the renal pelvis, calyces and renal column.There is no significant difference between left and right kidney in the ADC value, according to the measurement. So we take the average ADC values of two kidneys,and compare with the pathological grade by the patients who have done renal biopsy.And thak the value of serum creatinine into the formula (male:Ccr=(140-age) x weight (kg)/72×Scr; women:Ccr=(140-age)×weight (kg)×0.85/72×Scr) come out the value of endogenous creatinine clearance rate, analysis the relevance and accuracy of two exam results with pathological results.Results:The differences in ADC values by normal group versus grade Ⅲ-Ⅳ and grade Ⅰ versusⅢ-Ⅳ were all significant, as well as grade ⅢandⅣ,Ⅳ andⅤ respectively when b value was600s/mm2and900s/mm2.Furthermore pathological classification and the measured ADC values showed a significant negative correlation.And the differences in Ccr values by grade IV versus Ⅰ-Ⅲ,and grade V versus Ⅰ-Ⅳ were all significant.Conclusion:kidney ADC value can reflect the severity of IgA nephropathy more accurate than Ccr value...
