Objectives:To explore the pathogenesis,clinical features and surgical management ofChronic subdural hematoma(CSDH)。Methods:We selected312patients with CSDH from January2003to December2012according to research criteria.A retrospective study was performed on the312patients inthe last10years,evaluating the clinical characteristics,radiological findings, complications,surgical results and comparing the efficacy of two different surgical methods:burr-holeirrigation and closed drainage with rubber channel system(4mm), and burr-hole irrigationand drainage with soft channel system(10mm) by consecutive negative pressure.Results:87.8%of the patients had good recovery,10.3%of the patients showed nochange or worsened,and1.9%of the patients died.The rate of good outcome,the mortalityrate and the rate of reoperation in the group of burr-hole irrigation and drainage with softchannel system(10mm) by consecutive negative pressure were89.2%,1.3%and7.8%,lowerthan burr-hole irrigation and closed drainage with rubber channel system(4mm)83.8%,3.8%and16.3%.There were less complications and reoperation rate for closeddrainage with rubber channel system(4mm) comparing to drainage with soft channelsystem(10mm) by consecutive negative pressure.Conclusions:Comparing with burr-hole irrigation and drainage with rubber channelsystem(4mm) by closed negative pressure, burr-hole irrigation and drainage with softchannel system(10mm) by consecutive negative pressure is recommended.It issafe,efficience,performed simply,less complications,and suit for various age groups.
