Condylomata acuminatum(CA), a kind of benign proliferative disease defect by human papilloma virus (HPV) in the genital or around the anus(except the subclinical infections), has evolved into one of the most common sexually transmitted diseases(STD). Since the1970s, CA has become a health problem which attracts world-wide attention for its gradually increasing high morbidity. And the incidence of this disease has increased so considerably in recent years that it becomes the second fastest sexually transmitted disease in China. With its clinical characteristics of CA such as its rapid hyperplasia rate, high infectivity, and easy recurrence after remedy, the disease is extremely harmful to the patients and thus becomes one of the biggest concerns all over the world.The three squamous epithelial cells such as skin, mucosa and metaplasia from the human body, which is the only natural reservoir for HPV, are all very sensitive to the virus. It has been proved that a close contact of HPV with damaged squamous epithelial cells will make the later infected, and then some of the infected squamous epithelial cells or CA, may die or multiply abnormally. At present it is still not very clear how HPV can result in the abnormal increase of CA, and it may be caused by the increase of the division rate of cells as well as the decrease of the death rate of cells. TNF related apoptosis inducing ligand (TRAIL) has a direct effect on the apoptosis of tumor cells, virus infected cells and transformed cells. Caspase-3, also named dead protease, is the central part which is involved in apoptosis caused by various factors. As recent researches on Trail and Caspase-3were mainly from the perspective of tumor, the author of this thesis will try to find through research the expression of TRAIL and Caspase-3Protein in CA tissue and healthy prepuce tissue, and to analyze their relativity and significance in order to find out their roles in pathogenesis.Materials and Methodology1MaterialsThe30CA patients under research, including18men patients and12women patients who are from16to50(30.32_+7.18) years old, are all from Dermatology and Venereology Clinics in the First Affiliated Hospital of Zhengzhou University. And these cases, who all have typical clinical features, have been confirmed through histopathology and their course of the CA disease is from20to90(54.6±12.63)days. Besides, the other materials which are used by the author as comparisons in her experiment are the prepuces of20normal males from urological clinics.2Methodology and the Analysis of the ResultsImmunohistochemical SP method was adopted in this experiment to examine30cases of CA tissue and to analyze the expression of TRAIL and Caspase-3in prepuces of20males. And according to the instruction in the kit, a color of light yellow to brown granular deposits in cell membrane and cytoplasm can indicate the positive nature of TRAIL which is mainly located in prickle cell layers; while positive Caspase-3, which are distributed flakily or dispersedly in the middle or upper layers of prickle cells, shows from a color of light yellow to a color of tan. The following results are scored by the rate of positive cells and coloring intensity. For the rate of positive cells,0point for the percentage of10%or below;1point from11%to25%;2points from26%to50%;3points from51%to75%and4points for 76%or above. And for coloring intensity;0point for achromatic color;1point for light yellow;3points for brown; and3for tan. The final score is achieved by the product of these two kinds of scores:intensive positive is represented as3+from a score of9to12points; positive is represented as2+from a score of6to8points; weak positive is represented as+from a score of1to points:and negative if it is0point.3StatisticsAll the statistics in this experiment were analyzed by SPSS17.0and Chi-square test was used to examine two independent samples for the comparison of the difference between the positive expression rates of TRAIL and Caspase-3protein. Their expression levels were assessed with rank-sum test and the correlation between these two protein expressions in CA issue was analyzed by Pearson’s Contingency Coefficient Method with an inspection level of0.05(a=0.05).Results1. The expression of TRAIL protein in CA tissue and normal epithelial tissueThere are22cases of positive TRAIL expression in CA tissues of30cases, with a positive expression rate of73.3%(22/30) and an intensity of++~+++. In20normal foreskin epithelial tissue, there are4cases of positive TRAIL expression, with the positive expression rate as20%(4/20),and the intensity as+~++. The statistic significance of these two positive expression rates is revealed as (χ2=13.675, P<0.05) while the intensity rate is revealed as (H=14.319, P<0.05).2. The expression of Caspase-3protein in CA tissue and normal epithelial tissueThere are18cases of positive Caspase-3expressions in CA tissues of30cases, with a positive expression rate of60%(18/30) and an intensity of-~++. The expression of Caspase-3in normal epithelial cells is comparatively higher, with an expression rate of65%(13/20) and an intensity of+~++. There is no statistic significance of differences between the positive expression rates.(χ2=0.127, P=0.721, P>0.05), and the statistic significance of the differences between positive expression intensity is shown as (H=5.767, P=0.016, P<0.05).3. The analysis of correlation between the expressions of TRAIL and Caspase-3in CA tissue.There are22cases of positive expression and8cases of negative expression among the30cases of TRAIL expression in CA tissue, while there are18cases of positive Caspase-3expression and12negative Caspase-3expressions. Through the analysis by Pearson’s Contingency Coefficient Method, it is shown that there is a positive correlation between TRAIL and Caspase-3in CA tissue of30cases.(r=0.585, P=0.001, P<0.05)Conclusion1. The over expression of TRAIL in CA tissue may have an influence on the apoptosis of cells and also has a close link with the emergence and development of CA.2. The abnormal expression of Caspase-3in CA tissue may be connected with the morbidity of CA.3. Trail and Caspase-3which are positively correlated in CA tissue may be involved in the emergence and development of CA.
