BackgroundThyroid gland is the largest endocrine gland in the human body, and is the only endocrine organ that store their products in extracellular. The thyroid hormone is a substance needed by the human growth, and participate in the metabolic activity of human tissues and organs. Thyroid cancer accounts for about0.8%-1%of all cancers, and is the common malignant tumor in head and neck. In recent decades, the incidence of thyroid tumor grow faster than other solid malignancies, and ranked fifth in female malignancy, while the etiology molecular mechanism of the disease is not yet clear. The human tissue kallikrein enzyme (KLKs) is encoded by a multigene family, as the widely used tumor markers. The abnormality expression of KLK gene, is associated with the clinical and pathological features of cancer patients, by affecting tumor cell growth, invasion and metastasis. The gene that latest found and cloned in the gene family named KLK15gene, related to the occurrence and development of tumor cells. In order to further study the role of KLK15gene in thyroid tumors, in this experiment, we use reverse transcription PCR and immunohistochemical methods to explore KLK15gene and protein expression in thyroid cancer, benign thyroid tissue and relatively normal thyroid tissue next the nodules, and analysis of the relationship between the KLK15gene with clinical cases in thyroid cancer patients. Aim to provide a theoretical basis for the clinical treatment and prognosis of thyroid cancer, and to provide a new idea for the refractory thyroid tumors.ObjectiveStudy KLK15gene and protein expression in thyroid cancer, benign thyroid tissue, relatively normal thyroid tissue next to nodules(cut edge and nodal distance of2cm or more), and analysis the ralationship between the gene expression of thyroid cancer with clinical cases data, to explore the role of the KLK15gene in the tumorigenesis and the development process of thyroid cancer.MethodThe wax block tissue specimens from139thyroid tumor patients were collected from the First Affiliated Hospital of Zhengzhou University, between January2000to December2002, were all confirm the diagnosis by the pathologist, including78cases of thyroid cancer,32cases of nodular goiter and29cases of normal thyroid tissue. We use the reverse transcription PCR and immunohistochemical methods to explore the expression of KLK15gene and protein in different thyroid tissue, and applicate the statistical methods to analyze the relationship between the expression of KLK15gene and clinical case data of thyroid cancer. The data was analyzed by software SPSS17.0.ResultsThere are48positive KLK15gene expression (61.5%) in78cases of thyroid cancer tissue,9positive expression (28.1%) in32cases of nodular goiter,7positive expression (24.1%) in29cases of relatively normal thyroid tissue next to nodules. The expression rate differences of KLK15gene between the three groups was statistically significant, χ2=17.275, P<0.05. The KLK15gene expression is associated with the patient’s age, tumor size, and whether to invade surrounding tissue, lymph node metastasis, the occurrence of distant metastasis, and pathological types. For the thyroid cancer patients with KLK15gene positive expression, the average survival time is67months, while the average survival time of thyroid cancer patients with KLK15gene negative expression is128months, P<0.001, the difference was statistically significant. We use the multivariate Cox proportional hazards regression model method to analyse, the results show that, the KLK15gene expression (P <0.05), patients’age (P<0.05), lymph node metastasis (P<0.05), and pathological types(P<0.05) are the independent factors to predict survival rates of patients with thyroid cancer.ConclusionsThe KLK15gene involved in thyroid tumor occurrence, development and evolution of thyroid cancer, may be a cancer-promoting gene for cancer gene therapy clinical target. KLK15gene and protein is expected to be the biological indicators of thyroid cancer disease monitoring and prognosis.
