Myocardial Infarction Clinical Observation Of Combined Application Of Stem Cell Transplantation In Patients With Heart Failure

Purpose：Observation of stem cell transplantation in the treatment of postinfarction heart failure clinical curative effect.Material and method:The traditional Chinese Medicine Hospital of Liaoningprovince heart two branch and Liaoning province Jinqiu Hospital cardiovascularemergency center from2006August to2010June were in patients after acutemyocardial infarction combined with heart failure, A total of30cases, Selectedpatients were divided into two groups, including stem cell transplantation group19, control group11.Initial admission, routine conventional antiplateletaggregation, crown expansion, strong heart, diuresis, improving ventricularremodeling and other drugs, and to the emergency coronary angiography and PCIoperation.Transplant group in PCI after7±2days for stem cell transplantation,transplantation day in catheterization room sterile environment, establish thefemoral artery pathway, with guiding catheter line ischemia-related coronaryangiography, the micro catheter placement within the guiding catheter, througha microcatheter will separate the UCBMNCs suspension or PBSC suspension wasinjected into the infarcted vessel distal.Every3minutes, injection of2~3ml,balloon occlusion for3minutes.Observation of transplantation group before theoperation, after1years,2years, the longest5years and a comparison groupof1years,2years,5years of heart function.Specified by ultrasoundpractitioners, application of Philips SONO7500ultrasound heartbeat graphinstrument, the evaluation of left ventricular ejection fraction and regionalmyocardial segmental motion.Ultrasonic heartbeat by American Academy ofsynchrony in patients with left ventricular segmental motion integration method(161normal movement,2motion is abate,3akinesia,4contradiction movement),calculated before transplantation, LVEF, LVSd and LVDd numerical. Results：1. Stem cell transplantation group before the operation and followed up to1years when compared, LVEF (3.8+0.2)%, LVDd decreased (1.5+2.9) mm, andtransplantation follow-up to2years,5years when there is no obvious differencebetween the results of.2. The control group followed up to1years, LVEF decreased (0.5+0.1)%, theincrease of LVDd increases (0.4+0.3) mm, and worsening year by year.Conclusion：Stem cell transplantation therapy for postinfarction heart failure in patientswith LVEF and LVDd were improved significantly, and this method is safe andeffective in the long term.
