Clinical pathway is recommended as a tool of medical quality and medical costs around the world. It is able to effectively improve the quality of medical care, and protect the safety; it is also able to improve the efficiency of work, and reduce the costs of health care.Since October16,2009, Health Department has Issued357western medicine clinical pathways. Since January26,2011, State Administration of Traditional Chinese Medicine has issued304Traditional Chinese medicine (TCM) clinical pathways. The management of clinical pathway carries out comprehensivly.In this paper, type2diabetes mellitus and knee arthritis, two chronic common will be studied. Considering the costs and effectiveness, using comprehensive evaluation method, cost-effectiveness analysis, and other medical statistic methods, this paper will comprehensively understand the costs and the effect of the western medicine clinical pathway and TCM clinical pathway; objectively evaluate the economic effect between the western medicine clinical pathway and the TCM clinical pathway of these two diseases; sort out the main factors to affect the costs and the economic effect of the TCM clinical pathway and western medicine clinical pathway; found the problems existing in the current management of the TCM clinical pathway; targe ted ly put forward suggestions. The ultimate goal is to promote the management of the TCM clinical pathway.The results show that the average costs of the TCM clinical pathway of type2diabetes mellitus is slightly higher than the western medicine clinical pathway; the drug costs are higher than the western medicine clinical pathway; the effectiveness is no significant difference; the economic effect is not as good as the western medicine clinical pathway. The average costs of the TCM clinical pathway of knee arthritis is far lower than the western medicine clinical pathway; the diagnostic costs, treatment costs, drug costs and the materials costs are all lower than the western medicine clinical pathway, and the differences of the drug costs and the materials costs between two groups are the disparity; the effectiveness and the economic effect are all better than the western medicine clinical pathway.The influencing factors of economic effect includes three aspects:â‘ the difference of treatment means;â‘¡the length of stay;â‘¢the companion diagnosticsFor the above findings, main advantages of the TCM clinical pathway are:â‘ the using of the TCM antidiabctic drugs effectivly reduces the using of insulin;â‘¡the TCM clinical pathway of knee arthritis is suitable for the elderly, infirm patients with the course is not too long;â‘¢the length of stay has been well controlled.The main problems of the TCM clinical pathway are:â‘ the TCM clinical pathways of these two diseases are not detailed;â‘¡the management of the entrance and the exit of the pathway need to be strengthened:â‘¢lack of internal incentive and restraint mechanisms.In order to implement the TCM clinical pathway more smoothly, the suggestions are:â‘ insisting on the advantages of the TCM clinical pathway in treating certain disease;â‘¡constantly adjusting the TCM clinical pathway;â‘¢focusing on the entrance and the exit of the pathway;â‘£establishing a more comprehensive assessment and oversight mechanisms;...
