The number of patients with osteoarthritis (osteoarthritis, OA) has increased in recent years,which has become the focus by doctors and patients.The discomfort and dysfunction of KOA severely affect the patients’quality of life.Under the guidance of traditional Chinese medicine,The acupotomology follows the same basic principles of surgery.The acupotomology achieved good results in clinical application.Although Needle-knife therapy has formed a relatively complete,standardized treatment and achieved good clinical efficacy since the invention in the treatment by tendon disorders,However,lack of in-depth study on the mechanism of acupotom’s effect at different state. We want to discuss the Acupotomology mechanism for the treatment of KOA.ObjectiveThe knee osteoarthritis multi-center RCT clinical studies have shown that needle-knife treatment KOA significant effect. In this study, the left legs of rabbits are fixed,which caused KOA model. Ligament mechanical changes in cartilage matrix, we try to find out the possible ways acupotomology chondrocytes mechanical signal transduction intervention in which the role of laboratory evidence for the clinical accupotomology by tendon disease.MethodsSix-month-old healthy New Zealand rabbits were randomly divided (with gender equality) into normal control group,model group,acupotomy group,and electro-acupuncture (EA) group.After modeling,acupotomy group and electro-acupuncture group were treated for three weeks,once every week in acupotomy group, and three times in electro-acupuncture group.testted rabbits’ behavior with knee level assessment method; Expression of apoptosis genes integrinβ1mRNA,was conducted by Real-time PCR test,and expression of apoptosis proteins integrinβ1,was conducted by Western blot test.We also tested the tension, creep and stress relaxation data of medial collateral ligament by Bose Electro Force3300protracted test machine.Results1.Behavioral results:the score points of model group, acupotomy group, EA group increased extremely compared with the normal control group showed an extreme difference (P <0.01); After treatment, the decline in three groups of points, in varying degrees, the acupotomy group integral decreased most extremely,the acupotomy group compared with model group there was an extreme difference (P<0.01); Acupotomy can effectively improve arthralgia activity, swelling and gait of joint in KOA of rabbits. 2.The results of Real-time PCR tests:after modeling, Integrin β1mRNA expression levels in the rabbit knee cartilage extremely reduced; compared with normal control group showed an extreme difference. After acupotomology intervention, it can make Integrin (31mRNA expression level rise, cmpared with electro-acupuncture,the improvement of the acupotomology was strong.3.The results of western blot tests:after Modeling, integrinβ1protein levels in the rabbit knee cartilage reduced abnormally,compared with normal control group showed a significant difference. Acupotomology intervention raised the integrin (31protein expression levels, and electricity for this regulation is no big impact. Results and Real-time PCR experimental results are basically consistent.4.The result of ligamentous mechanics:after Short-term brake,tensile mechanical properties of the model group and normal group MCL are significantly different, the maximum load and maximum displacement of significant is reduced in model after braking,ligaments are subject to varying degrees of damage,the change was not significant compared with the normal control group in breakage,but in the stiffness,there are no change before and after modeling. Braking also can significantly change the material properties of the ligament; in the experimental results, ligament stress, strain and energy loss compared with before are significant differences. Stress relaxation experiment results show that the model group ligament stress relaxation rate is significantly higher than the relaxation rate of the normal group, had a good effect on the relaxation rate of the model group ligament. In acupotomology group,impact of stress relaxation rate is not very different, acupotomology group and EA group can improve the rate of stress relaxation, and produce different degrees of stress relaxation properties of the ligament, but the four groups were not significantly different (P>0.05). Creep results show that after modeling, there is a clear upward trend in the creep rate of the model group, obvious difference compared with before modeling. After the intervention of the acupotomology, the creep rate is almost close to the level of the normal group, that is no significant difference;While there is a big difference compared with the model group, this suggests that acupotomology on the creep rate of the model has a certain degree of impact. Electro-acupuncture intervention, the creep rate of the ligament almost no significant improvement, less than the creep rate of the model group.Conclusion1. Acupotomology intervention can significantly improve the tensile properties of the MCL and viscoelastic material,activation ligament repair function,the mechanics of the knee in a balanced and stable state.2. Acupotomology not only can adjust the mechanical signals stimulate cartilage cells and raise the gene and protein expression levels of integrin β1,but also can improve cartilage matrix.
