The Efficacy For The Treatment Of Gluteus Maximus Musculoutaneous Flap Sacral Pressure Sores

Objective To investigate the clinical application of gluteus maximus muscleflap transfer for repairing sacral pressure sores．Methods Aug.2009to Jan.2012，for the7patients who was suffering from thesacral decubitus which were attended our hospital, including3males and4females;aged from29to63, with an average46.8years old; duration from1month to14years,with an average35.6months;6patients are paraplegia,4patients with significantinfection,1case with diabetes mellitus; bedsores areas were from3cm×3cm to13cm×11cm, after-debridement wound area were from3cm×3cm to20cm×11cm, the depthof the bedsores were to bone surface in7cases,3cases with single bedsores,4caseswith multiple wounds; initial surgical treatment of pressure ulcers in3cases, surgicaltreatment for two or more times in4cases,7patients were using the gluteus maximusmuscle flap repair, including1cases of combined with vacuum sealing drainage; usesuction drainage postoperative for5to7days, the patients should prone or lateral,stitched after10to14days.Results6of the whole cases of muscle flaps survived completely, The cuttingarea from10cm×8cm to25cm×24cm.One patient who suffered from paraplegia andmultiple surgeries left one crack about2cm behind in the proximal suture,with a smallamount of bleeding and exudate, without suppuration, improved after dressingchanges; the wound healing of I period in6cases.Postoperative follow-up12to41months, no recurrence, no recurrence, the texture, color,elasticity of the flaps are fine,the shape are fine also, the recovery of the function are satisfied, surgical resultsare also satisfied.Conclusion Sacral decubitus long history, often associated with infection,healing delayed, so the quality of patient’s life was low.According to the location andcharacteristics of bedsores,use gluteus maximus muscle flap can gain a good clinicalefficacy.1st, The gluteus maximus muscle flap with a good function of anti-infection,fewer postoperative complications, donor site can be a closed immediately,theseadvantages make it become a common repair method in clinical practice.2ndAadequate pre-operation design, proper design of the flap, and the strict asepticoperation is the key to a successful operation.3rdElaborate nursing is the importantfactor of decubitus ulcer recurrence prevention of postoperative.
