Objective: Ultrasound-guided biopsy to detect S100A4in series tissues of thyroidpuncture, explored its significance and discussed the correlation between the ultrasonicimage characteristics and the positive expression of S100A4in80cases of papillarythyroid carcinoma.We expect that using the ultrasound imaging and S100A4to evaluateeffectively the biological behavior and prognosis of papillary thyroid carcinoma andother thyroid diseases.Methods: After a detailed thyroid high-frequency color Doppler ultrasoundexamination, ultrasound-guided punctured into the tumor and took some biopsy samplesout.Then the various tissue samples from the thyroid will be detected the expression ofS100A4by reverse transcription-polymerase chain reaction (RT-PCR) andimmunohistochemical method.Compared the differences of clinical ultrasound imagingcharacteristics and the pathological data.Results:1.Most of the ultrasound image characteristics of thyroid papillarycarcinoma are solid, unclear boundary, hypoechoic, the microcalcifications are oftenaccompanied in the tumors.And the signals of blood flow internal and peripheral in thetumors are ofen rich and irregular. The lymph node metastases often showed as ovalhypoechoic, heterogeneity mass. The structure of lymph node hilus is often unclear,within some cystic areas.2. The results of immunohistochemical experiments showedthat S100A4expression in the papillary thyroid carcinoma was higher than that inatypical follicular nodular hyperplasia. And the nodular goiter showed no S100A4expression. The expression of S100A4in each matched metastasized lymph nodes wassignificantly higher than that in primary papillary thyroid carcinoma. And the results ofRT-PCR experiment were basically consistent with those of immunohistochemicalmethod.3. For80cases of papillary thyroid carcinoma, by analyzing the relationship between the sonographic features and the expression of S100A4, we found that S100A4positive expression is associated with whether the border clear or not, whether lymphnodes metastasis and microcalcification in the ultrasonographic image of thyroidpapillary carcinoma. The relationships between the tumor size, location, internal echo,blood flow classification and the expression of S100A4were not found. The ultrasoundimages of thyroid papillary carcinoma that showed the fuzzy boundaries, lymph nodemetastasis in the cervical part and microcalcifications in the mass expressed high levelof S100A4, and patients may have a poor prognosis.Conclusions:1.The overexpression of S100A4may be involved in a variety ofthe biological processes in thyroid papillary cancer, especially in the development oflymph node metastasis.2. Interventional ultrasound in biopsy techniques combined withthe detection of S100A4can be used to predict and diagnosis for the thyroid disease.S100A4is a very effective indicator for the preoperative diagnosis and prognosisassessment of thyroid neoplasm.3. To a certain extent, the sonographic features ofthyroid papillary carcinoma reflected the expression level of S100A4and valued theprognosis of biological behaviors.
