Objective: To reduce the incidence of carbamazepine/allopurinol inducedSJS/TEN,we want to find an accurate, simple, fast and economical HLA-B*1502andHLA-B*5801genotype detection method, make sure it could be widely used in clinicaltesting. Meanwhile, find out the frequency of HLA-B*1502and HLA-B*5801in southJiangsu Province, respectively.Methods:1.We had collected4cases with carbamazepine induced SJS/TEN,4caseswith carbamazepine induced MPE, and50cases without taking carbamazepine orallopurinol from south Jiangsu Province.2.Extract DNA from all58blood samples, thendetect HLA-B*1502and HLA-B*5801using PCR-SSP. Analysis the frequency ofHLA-B*1502and HLA-B*5801in south Jiangsu Province, respectively.3.Authenticate allsamples using PCR-SBT.4.Screen HLA-B*1502of all58samples using SNP detectionmethod.5.Screen HLA-B*1502and HLA-B*5801of all58samples using direct PCR-SSPfrom whole blood.Results:1.9cases of HLA-B*1502positive and4cases of HLA-B*5801positivewere detected in all58cases using PCR-SSP.4cases with carbamazepine inducedSJS/TEN are all HLA-B*1502positive,4cases with carbamazepine induced MPE are allHLA-B*1502negtive.5cases of HLA-B*1502positive and4cases of HLA-B*5801positive were detected in the50cases without taking carbamazepine or allopurinol in southJiangsu Province. The frequency of HLA-B*1502and HLA-B*5801are10%and8%,respectively.2. The results of PCR-SBT are the same with PCR-SSP.3. SNPs Rs2844682and Rs3909184have no association with HLA-B*1502.4. The results of direct PCR-SSPfrom whole blood are the same with PCR-SBT. It is feasible to detect abnomal bloodsamples with our whole blood PCR systerm.Conclusion: We have screened HLA-B*1502and HLA-B*5801in all58samplesusing PCR-SSP, PCR-SBT, SNP detection and direct PCR-SSP from whole blood. We suggest that use direct PCR-SSP from whole blood to screen HLA-B*1502andHLA-B*5801in clinical testing.
