[Abstract] Object To analyze the difference of primary biliary cirrhosis and primary Sjogren syndrome and primary biliary cirrhosis complicating Sjogren syndrom in clinical features, anti-60kD Ro/SSA multiple antigenic peptides (MAPs) detection rate and cytokines, explore potential mechanisms of PBC with SS. Methods MAPs were artificially synthesized. EILSA was done to detect anti-MAPs antibodies in sera of three groups of patients above, and analyzing the relations of anti-MAPs antibodies and clinical features. Cytometric Bead Array (CBA) was performed to detect levels of cytokines, such as IL-1β IL-12p70、IL-17A、TNF、IFN-γ、IL-2、IL-4、IL-6、IL-10、 TGF-β1. Results among three groups were compared. Result The incidence of recurring parotitis (p=0.0067) and needing water to swallow dry food (p=0.0078)were higher in the group of PBC with SS than pSS. ALP level was lower than PBC group(p=0.0269), TC level was higher than pSS (p=0.0017)and PBC (p=0.0185)group. There was no essential difference in clinical features, other liver function tests (ALT、 AST、ALP、GG、TBIL、DBIL、TBA、LDH) and the antibody profile between PBC and PBC with SS. The detection rate of anti-60kD Ro/SSA and anti-52kD Ro/SSA by ELISA were90.9%and57.0%respectively. Significant difference of anti-MAP20antibody detection rate was observed between PBC with SS and pSS groups (p=0.0073), and anti-MAP3,7and17antibody detection rates were similar to pSS, while anti-MAP20antibody detection rates was similar to PBC. The levels of sera IL-1β(p=0.0195)and IL-17A (p=0.0287)of PBC with SS were both greater than PBC, similar to pSS; while levels of sera IFN-y(p=0.0302)and IL-2(p=0.0370)were both lower than pSS, and the level of TGF-β1was lower than PBC (p=0.0094) and pSS(p=0.0283).Conclusions There was no significant difference in clinical features, biochemical tests and antibody profile between PBC with SS and PBC, but the injure of salivary gland of the former was milder. Some anti-MAPs detection rates of PBC with SS were similar to pSS or PBC, and the level of some systematic proinflammatory cytokines increased, while some antiinflammatory ones decreased, similar to pSS roughly.
