Objective:To evaluate procalcitonin clearance (PCTc) as a prognostic marker in sepsispatient in ICU. Method:150patients of sepsis were divided into two groups:survivalgroup(95patients) and dead group(55patients).The APACHE score,SOFA scores of sepsispatients were calculated on the24h after admission to the ICU.The serum PCT levels weremeasured at1d,5d,7d and9d after admission,then calculated the PCT clearance of5d,7d,9d.The serum PCT levels and PCT clearance were compared between two groupsand the ROC curve for predicting the prognosis of PCT clearance were analysed.Results:Procalcitonin clearance is higher in survivors than in non-survivors,withsignificant differences at7d ([69.69(-8.66-87.24)%vs [3.48(-470.96-58.70)] and9d([79.21(42.49-94.31)%]vs [-5.19(-428.85-70.42)]%,P<0.01).The area under the ROCcurve is0.69(95%CI,0.59-0.80,P<0.01) for procalcitonin clearance at7d and0.79(95%CI,0.70-0.89,P<0.01) at9d. Conclusions:ICU mortality is associated to sustainedhigh procalcitonin levels,suggesting that procalcitonin clearance at9d may be a morevaluable prognostic marker then7d.
