Objective:The study is aimed to examine the pre-operative and post-operative levels of alpha-fetoprotein (alpha-fetoprotein, AFP) and AFP heterogeneity (Lens culinaris agglutinin-reactive alpha-fetoprotein, AFP-L3) for patients with TACE treatment, to determine the relationship between the variation of the serum AFP-L3%, of AFP level and the efficacy of TACE surgery, and to explore the clinical value of AFP-L3%for HCC patients after TACE treatment.Methods:We collected from Run Run Shaw Hospital, Zhejiang University School of Medicine and The Sixth People’s Hospital of Hangzhou,28liver cancer cases with the TACE treatment and complete clinical data. All cases show positive AFP(>20ng/ml). After the separation of all serum, samples were frozen under-80℃in a ultra-low-temperature refrigerator. The variation in the tumor volume and the deposition of iodized oil were used as the judging criteria for efficacy, we use micro-centrifuge column method for the AFP-L3separation, and compare TACE surgery level of the pre-and post-operation. We employ the statistical analysis to the corresponding data.The results:All patients studied here had received TACE surgery. Based on changes in tumor volume evaluation criteria (RECIST criteria),28patients were divided into effective group and ineffective group, which has24patients and4ones respectively. Males predominate in both groups, This difference was not statistically significant (P>0.05). The average age for the patients in the effective group is lower than that of the ineffective group, but this difference is statistically meaningless (P>0.05).Among24effective patients,1week after TACE surgery, AFP level for9cases declines, AFP-L3%level for19cases declines. However,1month after TACE surgery, AFP level for14cases declines, AFP-L3%level for21cases declines. The difference between the two groups was statistically significant (P<0.05).Conclusions:The variations of AFP-L3%after HCC TACE treatment show different features compared to that of AFP. AFP-L3%decline forecast more meaningful with TACE efficacy compared to that of AFP, especially in the early (after1week) more advantages. Therefore, post-operative AFP-L3level is a significant independent factor for determining the TACE curative effect.
