ObjectiveThis study investigated for the impact of the systematic family intervention onself-management of school-age children with asthma, results of children inhalationtechnique,health status of children with asthma and their parents mental health status;with the aim that more children with asthma families will be able to accept thelong-term systematic asthma home management, in order to control the disease,reduce the incidence of asthma, and make the quality of family life of children withasthma better.MethodsThis study was collected76cases of children from6to14years old and theirparents,The childrens were diagnosed for Asthma by the third level Hospital of HunanProvincial pediatric asthma specialist outpatient from December2011to July2012.They were randomly divided into two groups, including the intervention group with38patients and the control group with38cases. The both groups received routineout-patient treatment and care during the study; in addition, the intervention groupmust accept the systematic family intervention. In this study, the general informationquestionnaire,drug inhalation technology assessment, children with asthmaself-management of childhood asthma control test (C-ACT) scal,the SymptomChecklist (SCL-90) were adopted;children with asthma self-managementskills(including before through a comparative study,4th month,7th month), technicalaccuracy of drug inhalation, the childhood asthma control test score, the number ofattacks, emergency department visits, hospital days, absenteeism time and the cost of treatment of asthma within three months, parents,mental health level were compared.The role of family intervention in school-age children with asthmaself-management,results of children inhalation techniqu,health status of children withasthma were collected at some Points and the parents mental health was analyzed andevaluated.Results1. Comparison results of self-management skills of Children with asthma: Priorto the study,self-management total scale score of children with asthma in theintervention group was117.67±7.88; management of daily living score got22.97±1.81; medical management of the disease score reached61.75±6.67; andpsychosocial management score was32.94±1.80. In the study of the seventh month,with the impact of the systematic family intervention, all kinds of scores of childrenwith asthma got better: total scale score was154.96±4.53;management of dailyliving score was31.70±1.52;medical management of the disease score was81.66±1.55; psychosocial management score was41.59±2.07; and the difference wasstatistically significant (P <0.05).2. Comparison results of children inhalation technique: Before the study, thedrug inhaled score of children with asthma in the intervention group was6.70±1.76;children who got more than9account for5.41; those who got from6to9account for72.97. In the study of the seventh month, with the impact of thesystematic family intervention,the drug inhaled score got better, which became9.32±1.13; those who got more than9account for70.27; those who got from6to9accountfor29.73. So the difference was significant statistically (P <0.05).3. Comparison results of C-ACT: Before the study,Children’s Asthma ControlTest (C-ACT) score of the intervention group was18.43±1.68. In the study of theseventh month, with the impact of the systematic family intervention, control test(C-ACT) score of intervention group got better, which was21.79±0.88; thedifference was statistical (P <0.05).4. With the impact of the systematic family intervention on health status of children with asthma, compared with the control group, The number of children whoattacks more than three times decreased from56.76%to21.62%; The children ofemergency visits more than three times decreased from51.35%to8.11%; Thechildren who stays in hospital more than seven days decreased from45.94%to5.41%;The children with absence time more than three weeks decreased from18.92%to5.41%; The children of asthma treatment who cost more than three thousand yuandecreased from37.84%to8.11%. So the difference was significant statistically (P<0.05).But the control group showed no statistical significance(P>0.05).5. Prior to the study, the factor scores of parents SCL-90: Somatization (1.84±0.34), coercion (1.63±0.20), interpersonal sensitivity (1.92±0.33), depression (1.91±0.28), anxiety (1.84±0.34), hostilities (1.81±0.29), pointsterrorist (1.83±0.30),paranoid (1.45±0.20), mental illness (1.31±0.18). Compared with the normal adult(norm),the difference of the factor,scores was significant statistically (P <0.05) exceptthe coercion,paranoid and psychotic symptoms.When it came to the7th month, theparents,SCL-90score excpet the coercion, paranoid, psychotic symptoms weredecreased,such as somatization (1.62±0.42), interpersonal sensitivity (1.68±0.31),depression (1.68±0.31), anxiety (1.60±0.36), hostility (1.52±0.30), terror(1.58±0.34); the difference was significant statistically (P <0.05).Conclusion1.Systematic family intervention can improve self-management skills of childrenwith asthma significantly.2. Systematic family intervention can improve the results of children inhalationtechnique.3. The systematic family intervention can improve indicators related to the healthstatus of children with asthma to some extent.4. Systematic family intervention can improve the mental health of the parents of children with asthma significantly.
