Objective:The performance appraisal system has been implemented into variousdegrees by community healthcare institutions in different areas inChongqing. It was not until the basic medicine policy was fully introducedto the primary healthcare institutions that operating conditions differs fromdifferent modes of performance appraisal system. The purposes of thisstudy was to investigate the impact of performance appraisal system on theimplementation of basic medicine policy in community healthcareinstitutions, to understand the implementation status of basic medicinepolicy, and to find out how to standardize the performance appraisal systemthat is good for the implementation of basic medicine policy, to achieve thegoal that not only reflect the public welfare but also mobilize theenthusiasm of the staff.Methods:Approaches of the study: literature research, questionnaire survey,in-depth interview and expert consultation. Literature materials included:articles from databases, relevant policy documents of China or Chongqing on basic medicine policy and interview records from health sector leaders,experts and responsible person of community healthcare institutions. Datawere mainly from: the National and Chongqing Health Statistics Yearbook,the monitoring and evaluation system of basic drugs, and questionnaires.Results:1. The development and current status of the performance appraisalsystem in China and other countries: the value of the performance appraisalsystem was verified; and historical experiences and development models ofsome successful countries and regions were studied.2. The implementation status and effect of basic medicine policydiffered from different modes of the performance appraisal system incommunity healthcare institutions in Chongqing: detailed information waslearned from analysis results of the investigation of residents andhealthcare workers from community healthcare institutions.3. A complete “performance management” mode had been establishedin Qianjiang district and it was the most suitable mode to achieve idealindex of basic medicine policy. This showed that to implement the basicmedicine policy fundamentally, we should pay attention to thecomprehensive process of performance managementConclusions and recommendations:We should establish and improve a scientific and reasonableperformance evaluation index system that favorable to the implementation of basic medicine policy. The health bureau of districts and counties shouldattach great importance to performance appraisal system in communityhealthcare institutions. To ensure the performance appraisal system is ineffect with strong operability, and to form a complete mode of performancemanagement. We should standardize the purchasing behavior and rationaluse of basis drugs through evaluation index, balance the development ofvarious services and not impair basic medical or public healthcare servicesafter the implementation of basic medicine policy. We should accelerate thecomprehensive reform of community healthcare institutions, let it playfully function of “six in one”, establish a basic medical care systemgradually, and strengthen the role of community healthcare institutions inguiding the rational use of basic drugs among residents.
