Purpose:Based on the survey of the doctor-patient relationship in Shenyang municipalhospital, this study made a comparative analysis of cognitive differences of thedoctor-patient relationship satisfaction between medical staff and patients, andanalyzed the current status of and reasons for the tense doctor-patient relationship andeffective way that can ease the tension between doctors and patients. In the same time,combining Seaz.Hollender’s joint participation model, this study put forward theconcrete measures that are suitable for Shenyang municipal hospitals to improvedoctor-patient relationship, thus improving patients satisfactory and promotingharmonious doctor-patient relationship.Methods:In this study, a questionnaire survey was carried out in three Shenyang municipalhospitals, a total of120questionnaires for each hospital, where60for doctors and60forpatients. This questionnaire includes the following aspects: the overall understanding ofdoctor-patient relationship, the physician-patient communication, medical ethics, thecognitive ways of improving doctor-patient relationship, the impact of the media onthe doctor-patient relationship, etc. Through non-participant observation and profoundinterviews, investigators communicated feelings of doctors and patients when fillingout the questionnaire and their opinions on doctor-patient relationship.Results:In overall assessment on doctor-patient relationship, there are55.6%of respondentsthink that the doctor-patient relationship is in the status of "disharmony" or "verydisharmony". Doctors and patients have significant difference (2=113.269,p<0.001)onthe cognitive of responsible role of doctor-patient relationship. Doctors hold that theresponsibility of doctor-patient relationship primarily be attributed to social factors andpatients, while patients argue that the responsibility be primarily attributed to doctors. In the investigation of the main difficulty in doctor-patient communication,doctors and patients have significant cognitive differences (2=28.873, p <0.001)on the main difficulty, where22.9%of doctors believe communication difficulties aremainly due to "way of communication", while48.7%of the patients consider to be"status". Cognitive of good doctor-patient interaction between doctors and patientshave significant difference (2=33.847, p <0.001), where32.9%of doctors believethat health care reform, medical ethics, etc. are all able to improve doctor-patientcommunication, while48.7%of the patients hold that the ability to understand caneffectively improve doctor-patient communication. In medical ethics investigation,there are no significant difference (2=2.315,P>0.05) between doctors andpatients on medical ethics issues. Doctors and patients both consider thatunreasonable prescription and indiscriminate checklist are the main performanceissues. In the survey of ways to improve doctor-patient relationship, there aresignificant cognitive differences (2=18.768, p<0.001) between doctors andpatients on the key elements of building a harmonious doctor-patient relationship,where34.7%of doctors believe that the key is to improve the health care system,while36.7%of the patients consider the critical factor to be increasing expenditure onhealth care. There are significant cognitive differences (2=18.768, p<0.05)between doctors and patients on ways to improve doctor-patient relationship, where28.8%of doctors believe that the main way to improve the doctor-patient relationshipis the sound security system and increasing government investment, while26.0%ofpatients believe that the main way is to strengthen the supervision of the hospital. Inthe survey "the impact of the media on the doctor-patient relationship", there are nostatistically significant difference (2=5.206, p>0.05) between doctors and patientson cognitive of the impact of the media on the doctor-patient relationship cognition. Conclusion:The survey finds that, in general, doctor-patient relationship is relatively tight.this study puts forward concrete suggestive measures from the following aspects:strengthening medical ethics, strengthening doctor-patient communication, enhancingthe quality of medical humanities education for medical students, strengthening thecontrol of health care costs, strengthening the supervision of hospitals, improvingpatients’ scientific and cultural qualities, continuing health care reform in-depth,guiding the media to coverage the news objectively and comprehensively, etc.
