Analysis On Survival Of Inpatients With Nasopharyngeal Carcinoma In Tumor Hospital Of Hunan Province From1992to2005

Objective:To analyze the survival situation of inpatients with nasopharyngeal carcinoma (NPC) hospitalized in Tumor Hospital of Hunan Province from1992to2005, and to provide the scientific reference for formulating prevention and control measures of nasopharyngeal carcinoma, for making the optimum therapy and for developing tumor prevention and control work.Methods:The survival data about inpatients with NPC hospitalized in Tumor Hospital of Hunan Province during the period of1992-2005were analyzed by SPSS15.0software and Excel2003. The life table was used to calculate the survival rate, and the analysis of factors influencing the survival situation was performed using Gehan’s generalised Wilcoxon test. In univariate analysis, the survival curve of each group was counted, and then they were compared by Log-rank test. In multivariate analysis, the factors influencing the survival time were analyzed through Cox proportional hazard regression model.Results:Totally4,600inpatients with NPC were followed-up, among them, there were1,740survival cases,2,819death cases caused by NPC,12death cases induced by other causes,24cases failed to be followed-up midway, and5cases completely failed to be followed-up. The follow-up response rate was99.89%, the data deletion and loss rate was38.65%. The one-year survival rate, three-year survival rate, five-year survival rate, ten-year survival rate, and fifteen-year survival rate were93.47%,76.50%,62.69%,44.26%, and38.88%, respectively. The median survival time was88.56months (7.38years). Univariate analysis by Log-rank test showed that the survival rate differed significantly among NPC patients with different genders, ages, occupations, educations, clinical stages, initial therapies and radiotherapy progress (P<0.05). Multivariate analysis by Cox proportional hazard regression model indicated that the main factors influencing the prognosis of NPC were ages, genders, occupations, clinical stages, and initial therapies. There were statistically significant differences in the survival rate among NPC patients with different ages, genders, occupations, clinical stages and initial therapies (P<0.05).Conclusions:As compared with the one-year, three-year, and five-year survival rates of hospitalized cases of NPC in the Tumor Hospital of Hunan Province during the period of1987-1991, those of NPC cases hospitalized in the hospital during the period of1992-2005were all increased in different degrees. According to literature reports at home and abroad, the one-year, three-year, and five-year survival rates of hospitalized cases of NPC in the Tumor Hospital of Hunan Province during the period of1992-2005had all reached or surpassed those of domestic hospitals at the same level.
