Objective:1.To investigate the clinical value of human papillomavirusdeoxyribonucleic acid (HPV DNA) testing and cervical lesions.2.Takeimmunohistochemical methods to study the expression of HPV L1capsid protein (HPVL1) and the clinical significance of HPV DNA combined with HPV L1in the prognosisof cervical lesions. To analyze the relationship between the two methods andpathological results.Methods:1.Totally114patients who had the results of positive High-riskHPV,pathology and Thin-prep liquid-based Cytology Test(TCT) is Atypical squameuscells of undetermined significance(AS-CUS) and above in Yijishan Hospital ofWannan Medical College gynecology clinic from January2011to July2012wereanalyzed retrospectively. Among114patients, the cases of pathological diagnosis ofcervictis,cervical intraepithelial neoplasia I (CINI),CINII/CINIII, squamous cellcarcinoma (SCC) were42,11,51,10,respectively.2.The expression of HPV L1in cervicallesions especially infected with common HPV types was detected byimmunohistochemistry method.3. To analyze the relationship between the subtypes ofHPV、HPV L1capsid protein and pathological results.Results:The positive expression rate of HPV L1was64.3%(27/42),54.5%(6/11),15.7%(8/51),0%(0/10) in cervictis, CINI,CINII/CINIII, SCC, respectively. There wassignificant differrence between cervictis and CINII/CINIII or SCC,between CINI withCINII/CINIII (all p<0.05), but no significance between cervictis with CINI, betweenCINII/CINIII with SCC (p>0.05). The most common HPV subtypes among the114pathients were HPV16, HPV52, HPV58, HPV18, respectively. In the single infection16、18subtypes of HPV L1protein negative rate in L1protein rate was low expression,but highly expression in52、58subtypes.And infected with HP52and HP58in cervical tissue, L1protein positive expression group finally confirmed by pathologyinflammation proportion L1protein expression was significantly higher then negativegroup (p<0.05).Conclusions:1. HPV DNA testing can understand the high-risk HPV associated withcervical cancer, play an important significance in cervical lesion early screening andpredict cevical lesions progress.2.HPV L1capsid protein expression will graduallydecreased with cervical lesions increased,the L1testing is useful in evaluation ofcervical lesions and progression risk of cervical dysplasia.3.HPV L1protein insingleinfection52、58subtype was highly expressed, and L1protein positive expression groupfinally confirmed by pathology inflammation proportion L1protein expression wassignificantly higher then negative group. The combination of both HPVL1and HPVDNA detection can be helpful in management and prognostic prediction.
