| Background Preoperative transcatheter arterial chemoembolization(TACE) on the efficacy of resectable hepatocellular carcinoma(HCC)remains controversial.Objective Using the method of meta analysis,This study aims tosystematically identify and summarize the effect of preoperative TACE forpatients with resectable HCC.Material and Methodology A systematic literature search ofdatabases (Medline、PubMed、 CNKI and CBM) to identify publishedstudies of TACE administered preoperatively as a neoadjuvant treatmentfor resectable HCC was undertaken. Studies were selected for evaluation ifthey were randomized controlled trials （RCTs） or ProspectiveNonrandomized controlled trials（NRCTs）. Results were expressed byrelative risk (RR) with95%confidence intervals (CI).Results According to the strict inclusion and exclusion criteria，7studys are picked out and come into Meta-analysis, involved4RCTs and3NRCTs, including567patients with264composing one group takingTACE and303making the controlled group（n-TACE）.Compared with curing liver cancer only by operation,the RR(95%CI)of the recurrenceone,three and five years after the operation are0.86(0.70，1.07),0.96(0.85,1.08),1.01(0.94,1.09)(p>0.05), All of above have no statisticalsignificance,which reflects that preoperative TACE can not reducerecurrence rate after operation.Compared with curing liver cancer only byoperation,the RR(95%CI)of the total survival rate one,three and five yearsafter the operation are1.07(0.98,1.19),1.13(0.96,1.33),1.09(0.81,1.47)(p>0.05), All of these have no statistical significance,which shows thatpreoperative TACE can not increase the total survival rate after operation.Conclusions The present data suggests there are no significantbenefits for one，three，five years recurrence rate and overall survival ofpatients with resectable HCC using preoperative TACE treatment. Far morewell-designed randomized controlled trials are demanded to furtherevaluate and prove the effect of preoperative TACE. |
