| Objective：Using the opinion of the mixed product compensation theory andcompensation mechanism design theory, and through the quantitative andqualitative research, literature analysis, the existing problems of thecurrent compensation mechanism in community health service (CHS)institutions were exposed. Then the corresponding suggestions were putforward.Research content：1. Study on the cognition and attitude from patients and medical staffabout implementing of zero-profit essential drugs system of CHSinstitutions in Chongqing.2. Study on the economic operation and economic compensationmechanism of CHS institutions in Chongqing. 3. Analysis of current compensation mechanism in CHS institutions4. Policy suggestions about improving compensation mechanism inCHS institutions.Method:By the method of the stratified random sampling, this studyinvestigated the information from125medical staff and891residents of14primary health care institutions in7districts and counties of Chongqing.The cognition and attitude from patients and medical staff aboutimplementation of zero-profit essential drugs system were studied byliterature analysis, questionnaire survey method and date analysis method.The assessment of implementation of zero-profit essential drugs systemand compensation mechanism from medical staff was studied throughinsider interviews. The economic operation and economic compensationmechanism of CHS institutions in Chongqing were studied from statisticalyearbook.Research results:The positive effect of implementing the compensation mechanism:the percentage of fiscal subsidy was the highest in the income structure ofCHS institutions; the outpatient cost per visit was decreasing because ofthe lower price of essential drugs; the idea of less importance of publichealth service has put right, basic public health service developed muchbetter than before. The current problems of compensation mechanism: the basic medical care and health service function of CHS institutions shrunk;the current staff incentive mechanism was not very effective; thepercentage of fiscal subsidy from local government was far formreasonable standard.; the relevant policy of medical insurance needs to beimproved immediately.Policy recommendations：1.Improve the operation ability of CHS institutions, the propagandafor zero-profit essential drugs system and the comprehensive reform ofCHS institutions should be spread, the function of CHS institutions needsto be defined clearly, perfect the list of essential drugs.2.Improve the salary system of medical staff, strengthen their abilityof providing comprehensive service.3.Divide the proportion of fiscal subsidy from all levels ofgovernment rationally.4.Improve the matching compensation policy of medical insurance. |
