Objective:Retrospective analysis the clinical data of the patients whohad been operated by Frontobasal interhemispheric joint Lamina terminalisapproach. To discuss the surgical method and technique of the Frontobasalinterhemispheric joint lamina terminalis approach. assess the adaptationdisease and relative merits.Method: Study32cases operated withFrontobasal interhemispheric joint Lamina terminalis approach of sellarregion and the third ventricle. Composing craniopharyngioma in13cases(40.6%), pituitary adenoma in7cases(21.8%), meningioma in9cases(28.1%), astrocytoma in2cases (6.2%), germ cell tumors in1case(3.1%).All these32cases were operated by coronary incision within thehairline.bone flap with the unilateral right forehead or bilateralfrontal.Separate interhemispheric through anterior cranial fossa to sellarregion.Resected the tumor in sellar region. In necessary, incision laminaterminalis and cut off the tumor in the third ventricle. Results:whole resection of the tumor in21cases (65.6%);9cases of craniopharyngioma (69.2%);4cases of pituitary adenoma (57.1%);7cases ofmeningioma (77.7%); case of astrocytoma (50%). Subtotal resection in10cases (31.2%):4cases of craniopharyngioma (32.3%);3cases of pituitaryadenoma (42.9%);2cases of meningioma (22.3%);1case ofastrocytoma(50%).Most resection in1case (3.1%):1case of germ celltumors. The complications:electrolyte disorder in19cases (59.3%);diabetesinsipidus in22cases (68.7%), but all get relieved after treatment;memorydecline in3cases (9.3%), recover within three months; hypopituitarism in15cases(46.8%)hydrocephalus in1case(3.1%);2cases died (6.2%);Conclusion: Frontobasal interhemispheric joint Lamina terminalisapproach provides a broad operation visual angle, reveals the nerve andblood vessel of the operation area satisfying, such as optic chiasma, anteriorcommunicating artery, anterior cerebral artery, lamina terminalis. With ahigh percentage of whole resection. So it is a good choice to cut off tumorswhich laocatie in sellar region and the third ventricle...
