The Study Of Transient Receptor Potential Vanilloid Type1’s Expression And Significance In Patients With Organic And Functional Diseases In Upper Gastrointestinal Tract

Objective:Through studying the expression of transient receptor potential vanilloid type1in functional dyspepsia (FD)、gastric ulcer(GU)、 duodenal ulcer(DU) patients and healthy control group of the lower esophageal mucosa、the gastric antrum mucous and duodenal bulb mucous with immunohistochemistry, while detecting the diversification of calcitonin gene-related peptide(CGRP)、vasoactive intestinal peptide to investigate the relationship between the TRPV1and benign diseases of upper gastrointestinal tract and the possible mechanism. Methods:Collected10patients with FD(group FD) who were diagnosed in outpatient department room of luzhou medical college according to the standard of Roman Ⅲ,10patients with GU(group GU),10patients with DU(group DU) who were diagnosed in digestive internal medicine outpatient department or gastroscopy room of luzhou medical college affiliated hospital,and10health people (group N)for physical examination who requested the endoscopy examination and got the negative result as the normal control group.Each patient was performed under gastroscopy respectively took one mucous membrane from the lower esophagus,one mucous membrane from the gastric antrum and one mucosa membrane from duodenal bulb respectively, each piece was about0.3×0.3cm. Every piece was detected the expression of TRPV1and the expression of CGRP、VIP with immunohistochemistry. Results:1The expressions of TRPV1in different diseases of upper gastrointestinal tract:(1)The level of TRPV1in the lower esophagus mucous membrane about the group N、the group FD、the group GU、the group DU were:26.46±5.40、43.65±5.51、28.24±3.70、25.88±9.90, the expressions of TRPV1in group FD were significantly higher than group N(P<0.05)、the group GU (P<0.05)、the group DU (P<0.05), there was no significant different expression of TRPV1between the other groups (P>0.05);(2)The level of TRPV1in the Gastric antrum mucous membrane about the group N、the group FD、the group GU、the group DU were:29.63±10.07、41.36±12.00、20.48±3.17、32.19±9.67,the expressions of TRPV1in group FD were significantly higher than the group N (P<0.05)、the group GU (P<0.05),the group DU (P<0.05),the expressions of TRPV1in group DU were significantly higher than group GU (P<0.05),the expressions of TRPV1in group N were significantly higher than group GU (P<0.05), there was no significant different expression of TRPV1between the group N and the group DU(P>0.05);(3)The level of TRPV1in the duodenal bulb mucous membrane about the group N、the group FD、the group GU、the group DU were:36.99±9.11、48.49±8.42、34.15±6.07、38.77±11.78, the expressions of TRPV1in group FD were significantly higher than the group N (P<0.05)、the group GU (P<0.05), there were no significant different expression of TRPV1between the other groups (P>0.05).2. The expressions of TRPV1indifferent place of upper gastrointestinal tract:(1)The level of TRPV1about group N in the lower esophagus, Gastric antrum, duodenal bulb mucous membrane were26.46±5.40、29.63±10.07、36.99±9.11, The duodenum’expressions of TRPV1were significantly higher than the lower esophageal (P<0.05);(2)The level of TRPV1about group FD in the lower esophagus, Gastric antrum, duodenal bulb mucous membrane were43.65±5.51、41.36±12.00、48.49±8.42,There was no significant different expression of TRPV1about group FD between each part of the group by statistical analysis (P>0.05);(3)The level of TRPV1about group GU in the lower esophagus、the gastric antrum、the uodenal mucous membrane were28.24±3.70、20.48±3.17、34.15±6.07,The duodenal bulb’ expressions of TRPV1were significantly higher than the lower esophageal (P<0.05)、the gastric antrum (P<0.05),the lower esophageal’ expressions of TRPV1were significantly higher than gastric antrum (P<0.05);(4)The level of TRPV1about group DU in the lower esophagus、the gastric antrum、the duodenal mucous membrane were25.88±9.90、32.19±9.67、38.77±11.78, The duodenal’ expressions of TRPV1were significantly higher than the lower esophageal (P<0.05).3The expressions of CGRP in different diseases of upper gastrointestinal tract:(1)The level of CGRP in the lower esophagus mucous membrane about the group N、the group FD、the group GU、the group DU were:19.81±4.66、23.9±1.46、16.08±5.24、15.74±5.76, the expressions of CGRP in group FD were significantly higher than the group N (P<0.05)、the group GU (P<0.05)、the group DU (P<0.05),there was no significant different expression of CGRP between the other groups(P>0.05);(2)The level of CGRP in the Gastric antrum mucous membrane about the group N、the group FD、the group GU、the group DU were:18.67±6.17、24.41±2.88、16.93±5.09、17.81±4.08, the expressions of CGRP in group FD were significantly higher than group N(P<0.05), the group GU (P<0.05)、the group DU (P<0.05), there was no significant different expression of CGRP between the other groups (P>0.05);(3)The level of CGRP in the duodenal bulb mucous membrane about the group N、the group FD、the group GU、the group DU were:16.74±6.07、21.88±2.95、16.82±6.05、12.14±3.82, the expressions of CGRP in group FD were significantly higher than group N (P<0.05)、the group GU (P<0.05)、the group DU (P<0.05),the expressions of CGRP in group GU were significantly higher than group DU (P<0.05),the expressions of CGRP in group N were significantly higher than group DU (P<0.05),there was no significant different expression of CGRP between the group N and the group GU.4The expressions of CGRP in different place of upper gastrointestinal tract:(1)The level of CGRP about group N in the lower esophagus、the gastric antrum、the duodenal bulb mucous membrane were19.81±4.66、18.67±6.17、16.74±6.07, There was no significant different expression of CGRP about group N between each part of the group by statistical analysis (P>0.05);(2)The level of CGRP about group FD in the lower esophagus、the Gastric antrum、the duodenal bulb mucous membrane were23.91±1.46、24.41±2.88、21.88±2.95,the gastric antrum’ expressions of CGRP were significantly higher than the duodenal (P<0.05);(3)The level of CGRP about group GU in the lower esophagus、the gastric antrum、the duodenal bulb mucous membrane were16.08±5.24、16.93±5.09、16.82±6.05, there was no significant different expression of CGRP about group GU between each part of the group by statistical analysis(P>0.05);(4)The level of CGRP about group DU in the lower esophagus、the gastric antrum、the duodenal bulb mucous membrane were25.88±9.90、32.19±9.67.38.77±11.78, the gastric antrum’ expressions of CGRP were significantly higher than the duodenal bulb(P<0.05),the lower esophageal’ expressions of CGRP were significantly higher than the duodenal bulb(P<0.05).5The expressions of VIP in different diseases of upper gastrointestinal tract:(1)The level of VIP in the lower esophagus mucous membrane about the group N. he group FD. the group GU. the group DU were:24.23±12.11、38.53±4.29、34.32±2.32、31.99±6.97, the expressions of VIP in group FD were significantly higher than the group N (P<0.05). the group GU (P<0.05), the expressions of VIP in group GU were significantly higher than group N (P<0.05), there was no significant different expression of VIP between the other groups (P>0.05);(2)The level of VIP in the Gastric antrum mucous membrane about the group N. the group FD. the group GU. the group DU were:22.98±9.76、30.60±7.41、21.58±6.44、22.06±6.86, the expressions of VIP in group FD were significantly higher than the group N (P<0.05). the group GU (P<0.05). the group DU (P<0.05), there was no significant different expression of VIP between the other groups(P>0.05);(3)The level of VIP in the duodenal mucous membrane about the group N、group FD. the group GU、 the group DU were:20.08±5.89、26.42±5.36、17.60±5.70、17.00±4.75, the expressions of VIP in group FD were significantly higher than the group N (P<0.05)、the group GU (P<0.05)、 the group DU (P<0.05),there was no significant different expression of VIP between the other groups (P>0.05).6The expressions of VIP in different place of upper gastrointestinal tract:(1)The level of VIP about group N in the lower esophagus、the gastric antrum、the duodenal bulb mucous membrane were24.23±12.11、22.98±9.76、20.08±5.89,there was no significant different expression of VIP about group N between each part of the group by statistical analysis (P>0.05);(2)The level of VIP about group FD in the lower esophagus, Gastric antrum, duodenal bulb mucous membrane were38.53±4.29、30.60±7.4、26.42±5.36, the lower esophagus’ expressions of VIP were significantly higher than the gastric antrum (P<0.05)、 the duodenal bulb (P<0.05), the gastric antrum’ expressions of VIP were significantly higher than the duodenal bulb(P<0.05);(3)The level of VIP about group GU in the lower esophagus、the gastric antrum、the duodenal bulb mucous membrane were34.32±2.32、21.58±6.44、17.60±5.70, the lower esophagus’ expressions of VIP were significantly higher than the gastric antrum (P<0.05)、 the duodenal bulb(P<0.05);(4)The level of VIP about group DU in the lower esophagus、the gastric antrum、the duodenal bulb mucous membrane were31.99±6.97、22.06±6.86、17.00±4.75, the lower esophagus’ expressions of VIP were significantly higher than the gastric antrum (P<0.05)、the duodenal bulb(P <0.05),the gastric antrum’ expressions of VIP were significantly higher than the duodenal bulb(P<0.05).7The relationship of the expression of TRPV1:(1)The relationship between the expression of TRPV1and CGRP:by the analysis of linear correlation, the relationship between the expression of TRPV1and CGRP was significantly positive correlation in the group N(r=0.439, P<0.05),the relationship between the expression of TRPV1and CGRP was positively correlation in the group FD、the group GU、the group DU(r=0.302、0.198、0.035), but it was without statistical significance (P>0.05).(2) The relationship between the expression of TRPV1and VIP:by the analysis of linear correlation, the relationship between the expression of TRPV1and VIP was positively correlation in each group (r=0.175、0.021、0.143), but it was without statistical significance (P>0.05). Conclusion:1. All of the normal healthy people’s mucous membrane in the lower esophagus, gastric antrum, duodenal bulb expressed TRPV1, and the highest was duodenal bulb, the second was gastric antrum and the lowest was esophagus, the expression of TRPV1in duodenal bulb was extraordinarily higher than lower esophagus;2. The patients with functional dyspepsia (FD)’expressions of TRPV1in each place were significantly higher than the normal healthy people and the patients with gastric ulcer, the patients with FD’expression of TRPV1in the lower esophagus and the gastric antrum were significantly higher than the patients with DU;3. The expression of TRPV1in gastric antrum on the patients with gastric ulcer (GU) is significantly lower than normal healthy people, and is significantly lower than the patients with FD and DU, and the duodenal bulb mucous’ expressions of TRPV1on the patients with GU is the highest, the gastric antrum is the second, the lower esophagus is the lowest;4. The expressions of TRPV1in gastric antrum on the patients with duodenal ulcer(DU) were significantly higher than the patients with GU and the expression of TRPV1on the patients with DU is similar to the normal people;5. There is a significantly association between TRPV1and CGRP in normal healthy people,the TRPV1may be associated with VIP in normal healthy people,and the TRPV1may be associated with CGRP、 VIP in patients with FD、 GU and DU.
