Metaphor is the basic attribute of thinking mode for human beings, which provides fundamental mode of practice, spirit and culture for them. All creative resultant and cultural achievements are directly or indirectly marked on metaphor. Therefore intensive study on any culture cannot and should not be carried on without combined application of metaphor theories.Metaphor in Huangdi Neijing and Shanghan Lun is studied, from the perspective of cognitive metaphor, and is classified into nature metaphor, society metaphor, philosophy metaphor and religion metaphor. The allusive mode of thinking and linguistics are analyzed to prove the theories that the allusive thinking mode is just the analogy thinking mode in traditional Chinese medicine (TCM), that thinking mode and language is the origin of the comprehension and translation of TCM. The translation of TCM is thoroughly analyzed in this paper from perspective of cognitive metaphor to reach the following points.First is that metaphor, the common thinking mode of human beings based on similarity, is just the analogy thinking mode in traditional Chinese medicine, therefore only metaphor thinking mode originated in metaphor in TCM is studied deeply, the profound culture can be transmitted. For the proper translation must be based on precise comprehension of metaphor, it is the key to transmitting different cultures. Second is that on the basis of achievements acquired, the classification of metaphors (nature metaphor, society metaphor, philosophy metaphor and religion metaphor) is helpful to enhance the study on metaphor in TCM classics. Third is that experts from different and related fields are expected to work together for an excellent translation to spread culture of traditional Chinese medicine.The limitation of this paper is that the study is confined to thinking mode and linguistics of metaphor rather than multidisciplinary so that it is not deeply explained for articles, like Daigou to metaphor pulse condition, time measurement, like Jia, Yi day, ways to name organs with religious history in ancient times; while the deep logic of metaphor in TCM has not been to analyze systematically and thoroughly due to restricted documents and time.
