Hypothalamic Analgesic Mechanism Of Acupuncture Balanced Anesthesia Sacrococcygeal For Pain Threshold

Objective:according to the theory of acupuncture and the characteristic of anorectal surgeries which only need anesthesia in sacrococcygeal region,it will decrease the various complication by the integration of electroacupuncture and the less dose of sacral anesthesia method in the previous research.Establishing the sacrococcygeal imflammation-pain rat model,observing the chages of hyperalgesia scores.Exploring the evidence of theory that the generation and the release of POMC and (3-EP can reach the anesthetic effect by stimulating yaoyu point and changqing point by the interation of electroacupucture and anesthetic.Methods:40healthy adult male SD rats, weighing180-220g, were randomly divided into four groups:model group, anesthetic group, electroacupuncture(EA) group, electroacupuncture interated with anesthetic group. All rats were injected with50μL CFA in the region of left grion near the lateral skin of anus.It emerged swelling in the local injection point after the first day.It is a lateral sacrococcygeal inflammation-pain model and the hyperalgesia scores would be measured compared with the scored which would get after treatment. All groups dealt with different methods in the second day.Electroacupuncture interated with anesthetic group:yaoshu point and changqiang point were injected with50g/L lidocaine2.5ml/kg and stimulated by the XS-998B type of electroacupuncture apparatus.Adjusting the frequency of the apparatus into2/100Hz and the initial electric current into1mA,increasing1mA per10min until3mA,the total time is30min.Electroacupuncture group:Giving the same electric stimulation compared the electroacupuncture interated with anesthetic group;anesthetic group:Giving the twice dosage of anethethic compared with electroacupuncture interated with anesthetic group.model group:Only giving the mimic bundled action.After this experiment,POMC and (3-EP are measured in hypothalamus and serum with ELISA.Result:1.After treatment,the hyperalgesia scores was improved in all treatment groups.It has a best anesthetic effect in the elctricoacupuncture interated with anesthetic group.(p<0.05).It showed there is no difference between the anesthetic group and the electroacupuncture group.(p>0.05).2.It elevates the POMC and β-EP significantly in all treatmenr groups in hypothalamus and serum of rats,especially in the electroacupunture interated with anesthetic group.There is no difference between the anesthetic group and the electroacupuncture group.Conclusion:the outcome of the study is that it induces central anesthesia by the integration of electroacupuncture and anesthetic.It has a better therapeutic effect compared with the only application of electroacupuncture or anethetic.This comprehensive method reaches the effect of anesthesia by activating the central opioid system,promoting the generation and release of POMC and β-EP.It will provide a new method and the evidence of theory for sacrococcygeal surgeries in the future.
