BackgroundPathological diagnosis is an important method to distinguish between different types of uterine leiomyoma and leiomyosarcoma. Immunohistochemistry is an important aspect of pathological diagnosis. In recent years, more and more researches of expressions of immunohistochemical index of P16, Bcl-2, P53, Ki-67, ER, PR in tumor tissue. Also there literatures report that expression of one or two indicators in uterine fibroids or uterine leiomyosarcoma. But so far we find no articles studied six antibodies in uterine fibroids or uterine leiomyosarcoma in the same time. Also rare studies write on the diagnostic value of each antibody.ObjectiveExplore expression and its intensity of P16, Bcl-2, Ki-67, P53, ER, PR antibodies in leiomyoma and leiomyosarcoma, and diagnostic value of each antibody and their each two joint ones. Study relationship of those six antibodies with prognosis of uterine leiomyoma and leiomyosarcoma patients.Methods41cases of benign uterine leiomyoma tissue samples and14cases of uterine leiomyosarcoma tissue samples with complete data are collected which are resected in Fujian Medical University People’s Hospital during January2004and July2013. Collect clinical data and follow-up in order to make sure whether patients are recurred and dead. Using immunohistochemical detection method to determine expressions of P16, Bcl-2, Ki-67, P53, ER, PR. Using t-test,χ2test, rank correlation analysis, ROC curve, survival analysis methods in SPSS13.0statistical analysis software to do analyze work.Results1. There were no significant differences between expression of each antibody and recurrence, menopause, single or multiple.2. There was negative relationship between expression of Bcl-2and tumor size. For those patients with tumor diameter≥5cm, positive expression of Bcl-2was significantly lower than those with tumor diameter<5cm.3.There was positive correlation between age and P53in uterine smooth muscle tu mors. The older uterine fibroid patients were the higher P53expression was. 4. Positive expression rate of P16, P53, Ki-67, ER in the sarcoma group was significantly higher than fibroids group, P<0.05; Positive expression rate of Bcl-2in the fibroid group was significantly higher than sarcoma group, P<0.05.5. Diagnostic value of antibody:There were statistically significant diagnostic value of P16, Bcl-2, Ki-67and ER on uterine leiomyoma and leiomyosarcoma. Diagnostic value from high to low were Ki-67, ER, P16, Bcl-2. Diagnostic value of Ki-67and Bcl-2combined was higher than0.9.6. The median recurrent time of fibroid patients with uterusgroup was77months. There was no significantly relationship between recurrence and positive expression rate of six antibodies.7. The mean survival time of sarcoma group was15.7months. There was no significantly relationship between living state, living time and positive expression rate of six antibodies.ConclusionsHigh positive expression of P16, P53, Ki-67and ER suggested they had close relationship with uterine leiomyosarcoma. High positive expression of Bcl-2suggested they had close relationship with Uterine leiomyoma. In addition to P53and PR, the other four antibodies have some diagnostic value. Diagnostic value of Ki-67and Bcl-2combined was high.
