Analysis Of Surgical Treatments For Gallstones Combined With Common Bile Duct Stones

Objective: There are three conventional treatment methods about gallstonescombined with common bile duct stones，Comparison of clinical efficacy amongTraditional open cholecystectomy common bile duct exploration(OC+CBDE),laparoscopic cholecystectomy and laparoscopic common bile ductexploration(LC+LCBDE),and Laparoscopic cholecystectomy combined withEndoscopic Retrograde Cholangio-Pancreatography+Endoscopic sphincterotomy/endoscopic papillary balloondilata-tion+Endoscopic NasobiliaryDrainage(LC+ERCP+EST/EPBD+ENBD). Comparison of clinical efficacy amongLaparoscopic transcystic common bile duct exploration(LTCBDE), Laparoscopiccommon bile duct exploration and primary suture （LBEPS） and Laparoscopiccholedocholithotomy T—tube Drainage（LCHTD）.discuss the adaptation and operatingskills of（LBEPS）。Methods：A retrospective study about the collection of165patients with gallstonescombined with common bile duct stones who had Surgical treatment during Mar.2008to Mar.2013in our hospital. Separate these patients into group of OC+CBDE, group ofLC＋LCBDE, and group of LC＋EST. Compare the success rate,blood loss,operationtime, postoperative exhaust time, postoperative hospital stay,recent complication, thenumber of postoperative analgesia and forward complication among these groups.Separate the group of LC＋LCBDE into the group of LTCBDE, the group of LBEPSand the group of LCHTD. Compare the operation time, postoperative analgesicfrequency, postoperative hospital stay, biliary complications and total complication ratebetween these three groups.Result: For the operation time and Postoperative complications,OC+CBDE andLC+LCBED batter than LC+EST. For the blood loss, postoperative hospital stay,postoperative exhaust time the number of postoperative analgesia, LC＋EST group is better than LC+LCBDE group and better than OC+CBDE group. And for all the items,（LCHTD）is the best.Conclusion: LC+LCBDE is the preferred surgery for Gallstones with biliarystones.It is safe, feasible,effective and minimally invasive which has the advantage ofshort operation time,few complication and fast recovery,and LTCBDE is the bestchoose.LBEPS is a safe, feasible and ideal therapy which has much more advantagesaccording to the adaptation.
