| The study explores the application of automatic DNA ploidy analysis system in the early diagnosis of cervical cancer and the clinical significance in atypical squamous cell of undetermined significance (ASCUS) triage.141cervical samples were selected at random among120,000cases of cervical brush extract in Beijing from december from2008to2011.Two slides were prepared from each case:one slide was stained by papanicol aoustain for conventional cytology examination,while the other slide was stained by feulgen method for automatic DNA ploidy analysis.another HPV test was taken with moderate cytologic samples, the biopsy tissue pathology diagnosis and immunohistochemical p16staining. The conventional cytology inscreening CINâ…¡ or server lesion and the sensitivity, specificity, positive predictive value and negative predictive value of automatic DNA ploidy are analyzed (DNA ploid≥3), with the sensitivity being were40%and77.78%, the specificity66.67%and83.33%, the positive predictive value36%and68.63%, and the negative predictive value77.08%and88.89%. The detection of high risk type Human papilloma virus(HPV)(HPV16,52,58,68,18,31,33,35,39,45,51,56,59,66, CP8304,53)takes on the positive standard, with the sensitivity being86.67%, the specificity29.17%, the positive predictive value36.45%and the negative predictive value82.35%. Compared with the liquid based cytology, the DNA ploidy analysis system has higher sensitivity and specificity and is more statistically significant (P <0.05). Compared with the HPV, it has lower sensitivity but no statistically significance, higher specificity but statistically significance (P<0.05) which makes up for the high false positive rate of the HPV. It is found in the experiment that p16gene protein positive expression rate is gradually increases in normal cervical tissue, CIN â… , CIN â…¡, CIN â…¢ and cervical carcinoma. Especially in cervical carcinoma, p16protein positive expression rate is significantly higher than that in normal cervical tissue.At present the conventional cytology is the mainly used for cervical cancer screening in china. However, It is very difficult for clinicians to deal with ASCUS, It is found in this study that DNA ploidy analysis system is clinically significant in shunt. The application of automatic DNA ploidy analysis system can promote the positive detection rate in cervical carcinoma screening. P16can be used as a cervical intraepithelial non-neoplastic proliferative and neoplastic hyperplasia diagnosis marker and is a kind of effective method to distinguish between the CIN â…¡ and CIN â…¢. |
