Objective:High intensity focused ultrasound (HIFU) is a non-invasive method of solid tissue ablation therapy. This study aimed to compare the clinical benefits associated with the use of High Intensity Focused Ultrasound and chemotherapy in patients with advanced pancreatic carcinoma, and to evaluate the application of HIFU treatment in clinical.Methods:Sixty-six patients with advanced pancreatic carcinoma were enrolled and divided into HIFU group(n^32) and chemotherapy group(n=34) randomly. The therapeutic effects of two groups were evaluated in terms of Karnofsky performance status (KPS) scores, pain relief, serum CA19-9, and imaging by B-US and CT before and after the therapy. We also recorded median overall survival time and complications caused by the treatment.Results:The parameters between two groups showed no differences in age, gender, the stage and location of carcinoma(P>0.05). Compared with chemotherapy group, the therapeutic effects valued by KPS, pain relief, tumor markers, imaging by B-US and CT, median overall survival time of patients in HIFU group were superior(p<0.05). Pain relief, tumor markers and size in chemotherapy group were improved after therapy(p <0.05), while KPS score, tumor echogenicity and blood supply in chemotherapy group showed no change after therapy(P>0.05). No severe complications were observed after HIFU treatment, five patients had first-degree skin burns after the HIFU treatment, and they recovered without any treatment after2weeks. The complete blood count, urinalysis, serum chemistries, amylase, chest roentgenogram, and electrocardiogram showed no change after HIFU therapy.Conclusions:1.Compared with chemotherapy, HIFU treatment can increase KPS, relieve pain, decrease tumor growth and prolong the survival times of patients with unresectable pancreatic carcinoma.2. HIFU treatment is regarded as a safe method and has few side effects for unresectable pancreatic carcinoma patients...
