| Objective: Total hip replacement was gradually accepted by the patientsand doctors as an effective way to recover the advanced hip joint disease. Theindications of total hip replacement has been relaxed gradually with theprogress of medical technology. In recent years, the number of patients un-dergoing total hip replacement is gradually increasing. The causes, gender andage of patients are changing. Our research is to find the difference, develop-ments and changes between the10years.Methods: Statistical and analysis the patients undergoing total hip re-placement for the first time from2003.1.1to2012.12.31without the revisioncases in The third hospital of Hebei medical university. Record and group thenumbers, gender, age, etiology, surgical procedures, operative time, blood loss,postoperative bleeding, the type of the joint of the cases. Compare the groupsin ten years.Results: There were1733cases that conformed to the standards in thepast10years in our hospital,845cases of male,888cases of female. Refe-renced to ICD-10and discharge diagnosis, the cause were divided into12groups: avascular necrosis of the femoral head (702cases, male439, female263), femoral neck fracture (446cases, male142, female263), old femoralneck fracture (230cases, male116, female114), acetabular dysplasia (177cases, male39, female138), osteoarthritis (59cases, male35, female24),ankylosing spondylitis (44cases, male38, female6), bone tumor (32cases,male17, female15), rheumatoid (23cases, male9, female14), fracture ofacetabulum (9cases, male7, female2), intertrochanteric fracture (8cases,male3, female5), tuberculosis (2cases, male0, female2), congenital disloca-tion of the hip (1cases, male0, female1). The old femoral neck fractures were belonged to femoral neck fracture. Patients ranged in age from19yearsold to97years old. The average age of the patients was58.34, male55.07,women61.46. There were three kinds of hip prosthesis:1078cases of unce-mented component,564cases of cemented component and91cases of mixed.The average time of operation was shortened year by year. Bleeding dur-ing operation and postoperative bleeding were reduced year by year. Therewere three kinds of hip approach:72cases of anterolateral approach,137casesof aterall approach and1524cases of posterolateral approach.Conclusions:The number of THA patients increased year by year. Themain causes were avascular necrosis of the femoral head, femoral neck frac-ture and acetabular dysplasia. The first two causes growed rapidly. The fe-males were more than males in the proportion. Acetabular dysplasia was orig-inally a predominantly male disease. Ankylosing spondylitis was originally apredominantly female disease. The average age of patients was reduced grad-ually. The proportion of young patients was increased. The surgic-al biopsy technique was getting mature gradually. |
