| ObjectiveTo explore the clinical results of combined Wuwei Xiaodu Yin and the vacuum assisted drainage in the treatment of early infection after lower limb fracture treated with internal fixation which was associated with the type of toxic heat flaming.MethodsFrom February2012to June2013,42cases of early infection after lower limb fracture treated with internal fixation which was associated with the type of toxic heat flaming were treated in Fuzhou general hospital of Nanjing military region. They were randomly divided into treatment and control groups. The treatment group which had21patients, aged from23to60years old, average of42.81±11.72years old, included16males and5females, which included8cases of infection in femoral shaft and13cases of tibia and fibula infection. The control group which had21patients, aged from23to58years old, average of38.52±12.07years old, included15males and6females, which included6cases of infection in femoral shaft and15cases of tibia and fibula infection. Both groups were tested before surgery on white blood cell(WBC), C-reactive protein(CRP) and erythrocyte sedimentation rate(ESR), body temperature, bacteria culture and antimicrobial susceptibility testing. Both groups treated with debridement and negative pressure drainage, intravenous drip levofloxacin hydrochloride, select the sensitive antibiotics after susceptibility testing until two weeks after drainage tubes were removed. All in treatment group treated with Wuwei Xiaodu Yin for3weeks. All patents were followed up at least9months. Analysis WBC,CRP,ESR and body temperature after7days and14days. According to the standard of clinical disease diagnosis and improved cure to evaluate overall efficacy. Evaluate the infection recurrence rate after3and6months, evaluate the rate of fracture healing on3rd and9th months. All the dates were made statistical analysis by SPSS17.0.ResultsAll the patients were followed up for9-12months, average of10.8months. 1. There is no significant difference between the two groups in age, gender, the site of infection, body temperature and infection indicators(WBC,CRP,ESR)(P>0.05).2. Significant difference was detected between the two groups on body temperature in7th days(P<0.05); there is no significant difference between the two groups on body temperature in14th day(P>0.05).3. No significant difference was detected between the two groups on WBC in7th day(P>0.05); there is significant difference between the two groups on WBC in14th day(P<0.05).4. There is significant difference between the two groups in CRP and ESR in7th and14th days(P<0.05).5. There is significant difference between the two groups on bacteria culture in7th day(P<0.05), no significant difference was detected between the two groups on bacteria culture in14th day(P>0.05).6. The efficacy between the two groups has significant difference(P<0.05).7. There is significant difference between the two groups in infection recurrence after3and6months (P<0.05).8. There is significant difference between the two groups in fracture healing in3rd and9th months(P<0.05).ConclusionThe combined Wuwei Xiaodu Yin and vacuum assisted drainage in the treatment of early infection after lower limb fracture treated with internal fixation, it has advantages of effective infection control, improve antimicrobial efficacy, keep internal fixation, high negative bacterial infection recurrence rate, worthy of promotion. |
