Objective:In this study, topical recombinant human granulocytemacrophage colony-stimulating factor (rhGM-CSF) were applied to treatchildren with deep partial thickness facial burns, efficacy and safety of rhGM-CSF was observed and assessed during the treatment..Methods:(1) Grouping: Thirty children with deep partial thickness ficialburns hospitalized in our hospital from June,2009to June,2013were randomlyassigned to the treatment group and control group, with15children in eachgroup respectively. These two groups both followed the same treatment protocol.There are15children in the treatment group who were given rhGM-CSF. In themean time, there are also15children in the control group who were givenhydrogel matrix the same way as the treatment group. All of the patientscompleted the treatment processes.(2) Treatment: Routine debridement,disinfection and clean up the wound with sterile saline before given medication.the salves were evenly applied directly on wound1μg per square centimeteronce a day and then keep the record by the same person. In the hole process,both group were treated with exposure therapy, there were no other medicinewere used to promote wound healing. Debridement and symptomatic treatmentwould be performed if there was wound infection.(3)The percentage of healing area after given medication3、5、7、14days and the time of completed heal wererecorded and assessed. Changes during the treatment of infected wound wereobserved and evaluated. Wound effluent after treatment (purulent exudate),inflammation around wound after treatment and adverse reactions and allergicreactions during treatment were observed and assessed.Result:(1) The percentage of healing area in the treatment group werehigher than the control group after given medication5、7、14days (p<0.01,retrospectively); the time of completely healed in the treatment group (13.47+1.08) were significant shorter than control group(18.69+2.35),(p<0.01,retrospectively)..(2) Wound effluent: In the treatment group,6of them got2points,9of them got3points;in the control group,4of them got zero,8ofthem got1point,3of them got2points. During the hole process, the woundwere cleaner, the necrotic material were easier to remove, and the secretions,were significantly less than in the treatment group compared with the controlgroup.(3) Inflammation around Wound: In the treatment group,7of them got1point,8of them got2points; in the control group,9of them got zero,4of themgot1point,2of them got2points. The index of Inflammation around Woundwere higher in the treatment group.(4) There were11children in the firstdegree and4in the second degree in the treatment group; and there were3children in the first degree and12in the second degree in the control group.Which indicate that rhGM-CSF had higher security than hydrogel matrix.Conclusion: Topical recombinant human granulocyte-macrophage colony- stimulating factor gel can effectively shorten the healing time of children withdeep partial facial burns, accelerate healing, and reduce the incidence of burnwound infections;no adverse reactions occurred during treatment, whichindicate the high security of rhGM-CSF.
